  • The small amount of cement and other waste that had run off of the site and into the adjacent pasture has been removed in response to
   compliance assistance inspection of 2/15/11.  Waste was on top of snow/ice layer and did not appear to infiltrate the ground.  No 
   additional issues/concerns found.

Inspection revealed the following deficiencies: 1) Operator failed to notify surface landowners whose water supply was within 1000 feet of the well location prior to drilling. Plat listed one water supply within 1000 feet of the well with all others on public water. An additional water supply (well) was identified through this inspection approximately 350 feet from the gas well location that was not identified on the plat. Other water supplies are reportedly within the 1000 feet. A thorough review of the area must be done to identify all water supplies within 1000 feet of the gas well. These findings must be shown on a revised plat. This revised plat must be provided to all surface landowners or water purveyors whose water supplies are within 1000 feet of the well location. Revised plat must also be submitted to the Department. 605972

1964467 2011-04-11 No Violations Noted Notice of Violation issued on 3/3/11 for: 1) Operator failed to notify surface landowners whose water supply was within 1000 feet of the well location prior to drilling. Well has been drilled and stimulated.

Administrative compliance evaluation review has revealed that operator has identified all surface landowners whose water supplies are within 1000 feet of the well (13 additional water supplies were found). Operator has notified the 13 water supply owners, providing a letter and a new plat to each. Operator has provided the Department with the new plat, copies of each letter, and additions to the permit identifying the water supply owners and distance from well. With the after the fact notifications made to the water supply owners, and the documents submitted to the Department, compliance has been achieved.

No acclerated erosion or sedimeantion issues noted with site or access road. Well site successfully and uniofrmly restored with vegetation and rock. Drilling completed 2/11/22, Well completed 3/3/11. No violations noted at the time of this inspection
