2013842 2011-10-26 No Violations Noted Keyrock had contracted Phillip Drilling (Wayne Braden- Tool Pusher). E&S controls and well sign was present.

No freeboard or E&S issues at the time of inspection. Conductor pipe was installed to 23 feet (9 5/8¿ pipe) and sanded in. Surface pipe was installed to 397 feet (7¿ pipe). Surface pipe was cemented with 75 sks of Class A with 6 bbls return. Water was encountered at 850 feet. Well was drilled to TD at 1,046 feet. Phillip Drilling was in the process of moving off-site. Production pipe was not set. Phillip Drilling was waiting on Superior¿s wire line truck to log hole. Phillip Drilling was moving to Glass #10 well. DEP met Wayne Braden at the site. He ordered a snow plow truck to remove mud off township road.

2019502 2011-11-08 No Violations Noted At the time of inspection, site Id sign,ES plan and valid permit present. ES plan has been exposed to the weather;therefore, department reccomends it be replaced and also placed in a bag or other such device that will protect plan from becoming "wet."
Recommend operator add additional rock to shared access road due to muddy conditions, operator shall also continue to closely monitor and maintain in order to prevent mud from reaching township roadway. Brush truck is avaliable to ensure township road is kept clean.

Well drilled, not yet equipped for production. One below grade pit located onsite-lined, reccomend operator closely monitor pit levels as appears pit freeboard is approaching 2ft requirement. No accelerated erosion or sedimentation issues noted at this time. Site not yet temporarily or permanetly stabilized.
