Hence prior arrangements for restoration to PCSM standards have been made no violations are currently noted; although operator should continue to closely monitor and maintain site and access road until final permanent restoration is achieved as outlined in memo. Department will be notified up commencemnt of final restoration activities. Chevron EHS personnel, Curtis Small, OGI Rick Hoover, and Environmental Trainee Samantha Foulk on site during inspection.

Pad stabilized, adequately rocked with vegetative growth observed. No accelerated erosion or sedimentation issues observed. Wellhead surrounded by orange security fence. Well tag present, legible. Poly tank on secondary containment, water accumulation noted. Department recommends Operator empty containment as per current regulations.

Spoke to Chevron rep Curtis Small via phone on 7/8/2013 who informed Department that ditch above well site filled in, not needed due to topography, on-site conditions, and vegetative stabilization. At time of inspection, no potential erosion or sedimentation issues were observed as a result of this alteration. Department rep Samantha Foulk field-approved this change to plan.

Unable to adequately inspect access road and well site for erosion and sedimentation concerns due to snow-cover. Per previous agreement with Department, restoration activities were scheduled to commence March 30, 2013. A follow-up inspection will be conducted in the spring to ensure site is properly restored.
