  • There are concerns that this site requires an ESCGP-1 permit due to greater than five acres of disturbance included in this well site, access
   roads, other connected wells included in this project, etc..  Follow-up inspections will be conducted to determine the amount of total 
   disturbance, and to determine if the sites are in violation.  Culvert installed on access road requires rip-rap at the outlet to minimize erosion.  Please address this concern and contact this office upon completion.
