Site ID sign in place. E&S plan on site. Access road is stabilized with rock. No signs of accelerated erosion on access road. Access road entrance is rocked. No signs of accelerated erosion leaving well site. Frac pit appeared sound with proper compaction, embankment width, slopes, freeboard, etc.. Drill pit appeared sound. Freeboard was nearing the minimum 2' allowed. Contacted operator representative Ben Kissel about this as well as on-site personnell. Additional BMP's need to be installed in order to prevent any erosion/sedimentation issues, as reflected on the E&S plan on site. Spoke to Mr. Kissel about this who then requested an on site meeting

Site ID sign in place. E&S plan on site. Access road, site, and pits ok. Inspection conducted with operator representative Ben Kissel, site construction manager, and consultants in order to address concerns found during the inspection of this site on 10/27/09. Fluids had been removed from the drill pit to ensure 2' freeboard. Discussed additional BMP's needed to be installed in order to prevent any erosion/sedimentation issues, and to reflect the E&S plan on site. Mr. Kissel indicated that additional BMP's would be installed. Please contact this office upon completion.

Site ID sign and well tag in place. ESCGP-1 approval, permit and E&S plan on site. Access road was stabilized, with no signs of accelerated erosion. Inspection revealed the following deficiencies:

 1)  Failure to restore site and impoundment within 9 months of completion of drilling.  Large impoundment on site has not been backfilled and restored. Not permitted as centralized impoundment. Many disturbed areas around the site and production equipment are not vegetated.  Rock pad is sediment laden, and does not appear to be constructed of durable rock. 2) Failure to minize accelerated erosion, implement and maintain E&S controls, and stabilize site.  Nearly all of the silt fence that was installed is collapsed. Some of the BMP's shown on ESCGP-1 were not installed. Evidence of erosion/sedimentation found on banks and around equipment. Impoundment fluids must be removed and properly disposed of. Site, pad, impoundment must be properly restored.  Temporary BMP's mainta

2043141 2012-02-02 Outstanding Violations - No Viols Req'd During this inspection, well drilled and equipped for prodcution. Well sign ES plan present. According to records drilling completed on 10/25/09. Prevoius violations issed on 1/03/11 by WQS Curtis Small for failure to restore site and impoundment within nine months of completion of drilling. At the time of this inspection, "frac pond" still located on well site and currently full of water. Line of "frac pond" appears structurally compromised with various holes. Site has been seeded/mulch with minimal growth, does not appear to be at least 70% unfiorm vegeatative cover. Share access road, could be maintained as mud and rutting apepars to be a concern. Minor E/S issues noted on site due to lack of vegettative stabilization. No temporary bmp's observed.

Department is requested written response on what restoration efforts are being planned for i"frac pond" as well as time of when these events will commence.

During this inspection, no new violations noted previously issued violations ongoing.

2071675 2012-05-09 Outstanding Violations - No Viols Req'd During this inspection, well drilled and equipped for production. Well sign ES plan present. Two brine tanks located onsite located with metal secondary containment. Site has been seeded/mulch with growth.No accelerated erosion or sedimentation concerns noted at this time. According to records drilling completed on 10/25/09. At the time of this inspection, "frac pond" still located on well site and currently full of water. Liner of "frac pond" appears to have been repaired since last inspection- holes appear to have been patched. Since last inspection department noted settling of "sandy" material-possibly indicating flowback material??? Operator claims pit is being used for freshwater only. Department has a meeting planned for May 11 with operator to discuss outstanding violations at this location. Department had requested written response on what restoration efforts are being planned for i"frac pond" as well as time of when these events will commence. 2089729 2012-08-01 Outstanding Violations - No Viols Req'd At the time of this inspection, well ID sign, ES plan, ESCGP-1 present. Inspection follow up on reclamation and relining of impoundment as per agreement with department. Impoundment currently lined with minimal fluid, as operator actively pumping fluids out and hauling off site to an approval facility. Six roll off boxes located onsite in preparation for removal of impoundment liner and any contaminants that may have entered grounds of the commonwealth due to structural deficiencies in liner. At the time of this inspection, no new violations noted. A follow up inspection will be conducted when liner has been successfully removed and samples collected prior to relining impoundment for re-use as freshwater impoundment

2109943 2012-10-16 No Violations Noted At the time of this inspection, well ID sign, ES plan, ESCGP-1 present. Inspection follow up on reclamation and relining of impoundment as per agreement with department. It appears that impoundment has been successfully relined, minimal "fluids" present in freshwater impoundment, possibly that of precipitation. Areas previously disturbed have been seeded and mulch with observed growth. No accelerated erosion or sedimentation issues noted. It appears that operator has responded to previously issued notice of violation and no new violations are noted.

2136165 2013-01-31 No Violations Noted At the time of this inspection, well ID sign, ES plan, ESCGP-1 present. Drilling completed 10/25/09, well currently in production. As per previous agreement (w/ dept.) operator has relined freshwater impoundment. All previously disturbed areas on and surrounding well site have been permanently stabilized with rock and/or vegetative growth. No accelerated erosion or sedimentation issues noted at the time of this inspection. Recommend that operator remove water (as per regulations) from metal containment dike surrounding production equipment and brine tank. Shared access road stable. No violations noted at the time of this inspection.
