1781836 2009-04-06 No Violations Noted This inspection was performed as a Compliance Evaluation to a NOV that was issued on 2/9/09.

3/2/09- Written response was received from the operator. 4/6/09- Jarrod Shultz performed an inspection of the well site. The well site was equipped for production. The E&S plan has been revised to include any changes from the original plan. The filter fence has been repaired and/or replaced where it was needed. Additional E&S controls (silt fence, hay bales, and seeding & mulching) were added to the access road to assist in controlling Erosion and Sedimentation. With proper maintenance, the current E&S controls should properly limit erosion and sedimentation of the site and access road.

1817415 2009-07-31 No Violations Noted According to records, well completed on 2/5/09 to a depth of 5,037 ft and WRAC submitted on 4/28/09. Active permit since 9/30/09. Well head completed with 7¿ pipe reduced to 4¿ and reduced to 2¿ production line with 3 shut off valves. Production equipment consisted of heater, separator, data logger and 100 bbl tank. Line pressure on separator at 125 psi. No E&S issues and no IW issues during inspection surrounding the production well. The main access road to well will require additional E&S activity. Discussed topic with Penn View, Rick Blazek.
1817964 2009-08-03 No Violations Noted Drilling was completed on 2/5/2009. There are on E&S issues on the site at the time of inspection. Brine tank is in sound condition. Well is in production and site is restored. There is approximately 95% perrenial vegetative cover at the well site.
1965291 2011-04-13 No Violations Noted At the time of inspection, well drilled and in production. Well completed on 02/05/09. Well tag present. Site adequately and uniformly restored with >70% vegetative cover. Brine tank appears sound. No accelerated ES issues or IW issues noted on site.

Minor ES issues noted along shared access road due to new construction of Krause 1 site (Spud called for 3/30/11), which are addressed accordingly in Krause 1 site inspection; therefore operator should continue maintain shared access road accordingly.
