1894668 2010-06-24 No Violations Noted Based on record, permit was approved by 10/6/08. Well installed on 10/22/08 to a depth of 1390 ft. Report submitted on 11/6/08. Well completed with 8¿ pipe reduced to 4 ¿pipe reduced to 2¿ production pipe and 3 shut off valves. Production equipment consist of jack pump, 100 bbl brine tank ( < 10 bbl of fluid) and data logger (17.9 MCF daily, 14 psi static, 8237 MCF total). At the time of inspection, no IW and E&S issues.
1962224 2011-03-29 No Violations Noted At the time of inspection, well drilled and equipped for production. Well does not appear to be in production. Drilling completed 10/22/08. Production equipment appears rusty, recommend operator ensure structural integrity is not compromised due to rust. Brine tank ok, appears empty. Site and access road permanently restored. No accelerated erosion or sedimentation issues noted.

Note: Department unable to located annual production reports. It is recommended that operator resubmit reports, as annual production reports are required by regulation. At this time, department is unaware if well is/has been in production. A follow up admin inspection will be conducted to ensure production reports are submitted and well has/has not been in production in proceeding 12 months.
