2417938 Primary Facility 10/21/2015 Administrative/File Review In Compliance with Policy Admin inspt conducted in response to inspt 2417916 for reportable release of 5 bbl brine water.Upon receipt of spill report, Dept contacted Opr rep J.Canose via phone at 1602.Per Mr. C,release resulted from a leaking flange on brine tank.Mr. C stated that well-tender was last at well “a couple of days ago” and valve was not leaking at that time.Mr. C stated that tank has been emptied and moved.Excavated area is approx 6.5’x7’ and impacted materials are in roll-off box.Mr. C stated that no waterways have been impacted.Although release was originally reported as 5 bbl, Mr. C believes quantity released is less than 5 bbl based on area of impact and well-tender’s concerns that he may have miscalculated the tank readings.Mr. C intends to utilize a spill calculator to determine the quantity released.Dept is requesting these calculations be submitted to Department once quantity is determined.Opr appears to be ICP at this time.A FUI will be conducted to assess any environmental impact and/or issue any associated viols as necessary.

2444965 Primary Facility 2016-01-20 Administrative/File Review Outstanding Violations - No Viols Req'd Admin inspection to Inspection ID # 2435456. Operator is utilizing Act 2 for remediation of 2.5 bbl spill. Department contacted Operator Rep Jason Canose by phone to request update on remediation status. Mr. Canose stated that the final report was sent to ECB last week. Operator is awaiting response from ECB.

Department contacted ECB by e-mail on 1-25-16. ECB reported that the final report for this site was received on 1-15-16 and that a response will be issued within 60 days of that date.

2447517 Primary Facility 2016-02-03 Compliance Evaluation In Compliance with Policy Follow up inspection to Inspection Record #2419918 where violations were issued for a release of 2.5 bbl of brine.

Well tag present. Brine tank on site but appears unattached to well head. Site of release not backfilled. Security fence surrounding excavated area. No E&S issues observed at site. Operator is in compliance with policy at this time in so far as Operator has submitted NIR and a clean up report; currently being reviewed by ECB. A follow up inspection will be conducted at a later date to ensure adequate, permanent stabilization is achieved.

2465179 Primary Facility 2016-03-15 Administrative/File Review In Compliance with Policy Admin inspection to Inspection Record #2419918.

On 3-14-16 Department received written notification from the ECB Program that the Act 2 Final Report for this site was approved. Department recommends Operator ensure site is permanently stabilized with greater than 70% uniform vegetative growth upon backfilling disturbed area. A follow up inspection will be conducted at a later date to ensure site is backfilled, adequately vegetated, and permanently stabilized. Additional violations may be forthcoming pending inspection of site restoration/remediation.

2486683 Primary Facility 2016-05-18 Routine/Partial Inspection No Violations Noted Routine partial inspection.

At time of inspection security fence around area of release has been removed. Area of release is backfilled and appears to be seeded and mulched. Vegetation growing, however growth has not yet reached >70 uniform vegetative cover. No E&S issues observed on site. A follow up inspection will be conducted to ensure site is permanently stabilized. Previously issued violations for this site have been corrected.

2522873 Primary Facility 2016-09-23 Routine/Complete Inspection No Violations Noted At time of inspection no E&S issues observed on gated access road. No E&S issues observed on site.

Well appears equipped for production. Well tag present. Brine tank appears structurally sound. Area excavated due to a previous release appears to be vegetated. Brine tank placed on excavated area, however, site appears permanently stabilized.
