    various plats.  The correct permit number for OHM # 29 can be found above.  Please verify and make the appropriate changes.  The dirt dike that was constructed around the well has caused a large amount of water to pool up in and around the well head and supporting equipment.  You should consider draining this water into the vegetated adjacent field to avoid any issues/problems with the equipment.  A fairly substantial gas leak was noticed where the 2" feed line enters the ground, which should be inspected and repaired.  This has been referred to oil and gas inspector Rick Hoover.  Please address the above concerns.
