While perforating well for frac, had a misfire of perf guns while retrieving coil tubing. Vertical section of pipe was accidentally perforated at 4054 – 4056’, 4256 – 4258, 4417 – 4419’. Operator set CIBP at 6040. Set patches and cemented over each of the shallow perforated sections. Shut in pipe and tested for 24 hours. No pressure observed on outside annuli. Wells 1H and 3H have already been fracced and show now pressure on annuli. No gas bubbling was observed in the cellar. Operator will provide up date on progress of remedial work.

2530759 Primary Facility 2016-10-21 Routine/Complete Inspection No Violations Noted On 10/21/16, the Department conducted an inspection of the COG Carey R. Pad located in Harford Township, Susquehanna County. 3 Wells have been drilled on this pad and appear to be in production, the 1H, the 2H, and the 3H. The well pad and adjoining tank pad have good stabilization. There were 2 standing water puddles located on the well pad. Both puddles were tested with an Extech Exstik II Conductivity Meter. Both revealed measurements of 200 microsiemens. Next, the conductivity of the standing water within the production tank containment was measured and found to be at 600 microsiemens. There was approximately 3” of water in this containment. A Time Extension Request has been granted for the ESCGP for this site by the Department and will be effective until September 30, 2017. No violations noted.
2586414 Primary Facility 2017-04-20 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 14:35p.m.

The cellar was about half full of water with no bubbling noticed. The (13x9) annulus vent pipe was open and venting into a small plastic tank, the (9x5) annulus vent pipe was open and venting into a small plastic tank. There was 497psi on the production casing. I took a walk around the location and there were no obvious concerns noticed. I left the site at 15:10p.m.

2616434 Primary Facility 2017-07-18 Routine/Complete Inspection No Violations Noted Todays inspection was conducted to assess the site after having received notification of a nearby water supply complaint. Entire site is stable. Roadside ditches are in excellent condition. All 3 wells, the 1H, 2H and 3H all appear to be in production. No violations noted.

The well was in production. The (13x9) and (9x5) annuli vent pipes were connected together and venting to the same tank. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

The well was in production. There was water 6' below grade in the cellar with no bubbling noticed and there was no gas detected in the remainder of the cellar. The (13x9) and (9x5) annuli vent pipes were connected together and going to the same poly vent tank. There was no gas detected at the poly vent tank. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

2812681 Primary Facility 2018-11-29 Routine/Complete Inspection No Violations Noted Today's inspection was conducted as a routine inspection. No E&S issues were noted. Photo #4 shows the fill slope with a high number of shot gun shell casing present, suggesting site is utilized as a clay bird shooting location. No violations noted.
