2769040 Primary Facility 2018-08-17 Preoperation Inspection No Violations Noted On August 17, 2018, the Department performed an inspection of the CNX MOR40 well pad site. Present at time of inspection was Dave Federick, Spencer Johnson, Mike Farrah and Andrew Ryan (CNX). The site is located in Gray Twp., Greene Co. The inspection was related to request by the operator for a pre-operational inspection of the site, prior to the commencement of drilling operations on the site.

The operator has completed construction of the access road and well pad. Recent rainfall events had caused E&S BMPs to fail on the site. The operator has performed maintenance on BMPs and installed upgraded BMPs, which were redlined on the E&S plan on the site. A slope failure outside and down-gradient from the excess soil stockpile location has caused the stockpile to slide during construction. The sliding of the partially constructed stockpile has caused the silt sock in the area to fail. The operator has reestablished the silt sock and temporarily stabilized the area. 2776368 Primary Facility 2018-09-17 Routine/Complete Inspection No Violations Noted The well has been spud. A conductor pipe has been set and cemented. The operator is on site drilling well 059-27517.
2777615 Primary Facility 2018-09-12 Routine/Complete Inspection No Violations Noted On September 12, 2018, the Department performed an inspection of the CNX MOR40 well pad site. Present at time of inspection was Spencer Johnson (CNX). The site is located in Richhill Twp., Greene Co. The inspection was a routine inspection related to reported slide on the site by the operator.

The operator has completed construction of the site and has begun drilling operations. well pad. A slope failure outside and down-gradient from the excess soil stockpile location has caused the stockpile to slide during construction. The sliding of the partially constructed stockpile had caused the silt sock in the area to fail. The operator has reestablished the silt sock and has permanently stabilized the area. The operator has submitted a geotechnical report for the stabilized stockpile and slope as requested.

The operator notified the Department on 8/17/18 that Retention Pond #3 at the entrance has a cut slope failure. The slope failure has not impaired the function of the PCSM structure and is within the LO

2851928 Primary Facility 2019-02-19 Routine/Complete Inspection No Violations Noted On February 19, 2019, the Department performed an inspection of the CNX MOR40 well pad site. The site is located in Richhill Twp., Greene Co. The inspection was related to reported slides by the operator.

The operator has restored the site to the interim restoration plan and has constructed the PCSM BMPs. Slope failures in the cut slopes of the 3 Retention Ponds (PCSM) have developed. The failing cut slopes have been repaired by the operator previously, but continue to move. The slope failures have not impacted the functioning of the PCSMs at time of inspection, but the operator will need to stabilize the areas. The operator will need to complete a geotechnical investigation and report for repair of the slides and stability of the site.

5 wells have been drilled to completion on the well pad, and hydraulic fracturing operations are ongoing.
