2685233 | Primary Facility | 2018-01-19 | Preoperation Inspection | No Violations Noted | On Friday, January 19, 2018, I conducted a pre-operation inspection at the Don Flamenco Well Site located in Richhill Township, Greene County. The inspection was conducted as a result of an operator request in accordance with Section 3258(a.1) of Act 13, The Oil and Gas Act. Mr. Jamie Davis from EQT Production Company (EQT) was also present during the inspection. Due to excessive truck traffic (hauling stone for the well pad) on-site, the inspection was completed on Monday, January 22, 2018.
The Act 9 sign was clearly legible at the site entrance and all required documentation was available on-site. The access road and the well pad were in good condition and each still require more stone. At the time of inspection, stone was being hauled on-site to complete the well pad. After the well pad has been completed, additional stone will be added to the access road. All E&S BMPs have been installed and are functioning as intended. The site is located in Richhil Twp., Greene Co. The inspection was a compliance evaluation inspection of violations noted on December 4, 2018.
The access road entrance has been upgraded to a paved entrance. The operator has installed silt sock in the access road ditch line in an attempt to remediate the sediment discharge off site at the entrance. The remediation work at the entrance appears to be working. A second small unnamed trib to Long Run along the access road at a bridged stream crossing had been impacted by the run off from the access road. The operator has installed road breaks in the access road along the stream crossing to prevent sediment discharges of the site. Gray sediment observed in the unnamed tributary, along the access road, has been removed by the operator.
