Inspection Data

Inspections Performed

Inspection ID Inspection Category Inspection Date Inspection Type Inspection Result Inspection Comments
2672624 Primary Facility 2017-12-11 Routine/Partial Inspection No Violations Noted The Act 9 sign was in place and all required documents were present at the site. The site was well vegetated and there were no signs of erosion observed.

A meeting was requested by EQT to discuss adding a berm to the well pad. The intention of the berm is to prevent potential spills from leaving the well pad during drilling. Due to the impact the berm would have on stormwater control, this WQS advised EQT to contact Department environmental engineering staff.

2833395 Primary Facility 2019-01-22 Routine/Complete Inspection Violation(s) Noted On 1/22/2019 at approximately 1100, I conducted an incident response inspection of the Cosgray well site located in Franklin Township, Greene County in response to a reported spill of produced water.

On 1/21/2019 at 1400, a valve leak as a mist was noticed by the operator. The operator approximated that less than 40 gallons of produced water released from a GPU as a result. Specialized Professional Services (SPSI) responded to remediate the impacted area.

During the 1/22/2019 inspection, the following was observed:

  • Act 9 sign located at the site entrance was in good condition.
  • Some secondary containment mechanisms utilized on 4 GPUs but containment appeared unable to hold 110% of the unit storage capacity.
  • SPSI was on site during the inspection cleaning the GPUs.
  • Liquid observed in the grated tray between the GPUs.
  • Well identification tags were not able to be found on any wells. An oil sheen was observed in one well cellar (37-125-27404).
  • Production fluid tanks were within secondary conta
