2421467 Primary Facility 09/11/2015 Routine/Complete Inspection No Violations Noted On 09/11/15 at approximately 0830 hours I conducted a pre-op inspection of the Commonwealth 496 well site. All required documents were present. The Act 9 sign was present. All BMPs appeared to be in good working order. After my inspection I contacted Nick Staffieri to inform him that the pre-op had been conducted and that they were able to commence with their spuds. There were no violations noted at the time of inspection.
2525297 Primary Facility 2016-09-22 Routine/Complete Inspection No Violations Noted On 09/22/16 at approximately 1000 hours I conducted a routine complete inspection of the Commonwealth 496. The Act 9 sign was in place and all required documents were present at the site. The site was well stabilized and there were no signs of erosion observed at the time of inspection. There were no violations observed at the time of inspection.

On 12/08/16 at approximately 1000 hours I conducted a routine complete inspection of the Commonwealth 496. The Act 9 sign was in place and all required documents were present at the site. The site was well stabilized and there were no signs of erosion observed at the time of inspection. There were no violations observed at the time of inspection.

2567877 Primary Facility 2017-03-06 Routine/Complete Inspection No Violations Noted The Act 9 sign was in place and all required documents were present at the site. Along the left side of the access road an erosion rill was observed(see photographs). It is recommended that the rill be repaired once weather allows. The site overall was well vegetated and stable. There were no other signs of erosion of observed. The E&S controls appeared to be in good condition and functioning. There were no violations observed at the time of inspection.

2751658 Primary Facility 2018-06-04 Incident- Response to Accident or Event In Compliance with Policy On 06/01/18 at approximately 20:15 hours Chevron reported a spill of 5 to 10 gallons of soap. On 06/04/18 I, along with WQS Adam Ofstun, conducted an incident response inspection. The spill had been remediated by the time of inspection. The spill was addressed in compliance with the spill policy.
