2402551 Primary Facility 2015-08-06 Drilling/Alteration No Violations Noted HORIZONTAL MARCELLUS, Here is a response from the operator, Rick,as per our discussion on the evening of August 5th, 2015 we perforated from 218' to 220', attempted to establish an injection rate of 5 bpm. We were only able to achieve 2.5 bpm. We then subsequently perforated from 216' to 218'. On the morning of August 6th, 2015 we attempted to establish an injection rate of 5 bpm, and were able to do so with 40 bbls of fresh water. After 40 bbls of water were away we were only able to sustain an injection rate of 2.5 bpm.

We will woc 8hrs, we will then drill out to our cement to the FC @364' in order to Re-run a CBL log, if top of cement is below the basket we will perforate @268.The bond log shows little cement behind pipe. The operator decided to set cement plug in 20"casing and move to #3 well. The future plan is to P/A # 4 well pryor to drilling new # 4 well.

On 04/27/16 at approximately 0830 hours I conducted a routine complete inspection of the site. The Act 9 sign was in place and all required documents were present in the mail box.

The site overall was well vegetated. One location to the right side of the access road at the pad is recommended to receive additional seeding and mulch (see photos). There were no violations noted at the time of inspection.
