0299768196 2015-07-15 Plug / Plug Back No Violations Appalachian Well Surveys on location to tag bottom plug, run bond log, and shoot 4.5 casing off. The bottom plug was tagged lower than expected at 2400, but still covered the top of the uppermost perforations. No gas was present on the 4.5 casing. Top of cement was found at 1585 and the 4.5 casing was shot off at 1546. Cardinal Well Services pulled to joint to ensure the casing was free and Formation Cementing established circulation and pumped a 200 rip plug from 1500-1300 using 54 sks 1% gel cement, 100 lbs CaCl, 30 lbs LCM, and 20.6 bbls freshwater displacement. The 4.5 casing was then pulled to 489, circulation was established, and a 200 bottom of surface casing plug was set from 489-289 using 56 sks 1% gel cement, 100 lbs CaCl, 30 lbs LCM, and 4.5 bbls freshwater displacement. The 4.5 casing was then pulled to 180 and a surface plug was circulated to surface using 56 sks 1% gel cement and 20 lbs LCM. The cement did not drop below 30 below grade level.
0513464728 2004-04-16 Production Wells No Violations Inspection this day found well with jack, tank newly painted, IDs faded but there, no meter in run, not sure if producing or meter has been moved. Will contact operator and enquire - very nice paint job for a well not in production?
0679759943 2015-07-14 Plug / Plug Back No Violations On location to witness plugging operations for this well. Cardinal Well Services service rig is standing over the well. This well was plugged back to 2750 with a cast iron bridge plug. Cardinal Well Services used a 4 bit on their jar and stem to beat the bridge plug down to 3200, exposing the top of the original perforations. 1.5 tubing was set at 3160 and a bottom plug was set from 3160-2860 using 25 sks class A cement, 10 lbs LCM, and 7.2 bbls of freshwater displacement. Tubing was then pulled to 2450 and a second bottom plug was set at the top of the newer perforations from 2450-2200 using 20 sks class A cement, 10 lbs LCM, and 5.5 bbls of freshwater displacement. Circulation was not established on these plugs due to open perforations in the 4.5 casing. 1.5 tubing will be tripped out of the well and the remaining plugs will be set through the shot 4.5 casing. The bottom plug will be tagged tomorrow morning before shooting the 4.5 casing off.
1143808702 2016-05-24 Final Restoration No Violations The purpose of my inspection today is a status check on a final restoration. During my inspection I witnessed one 100 bbl. tank. Location has not met 1509.072 B requirements. Production equipment needs removed before the final restoration can be passed.
1864421065 2015-12-31 Plug / Plug Back No Violations Upon inspection, it was observed that the pit has been filled and the location has been graded. There is still a 100 bbl production tank and steel flow lines present on the location. There is a seed rake and a round bale on location, indicating that the operator plans on seeding and mulching the location.
2022382963 2015-07-08 Plug / Plug Back No Violations On location in response to a request from the plugging contractor, Kroll Energy, to inspect tubing that came out of the well for suitability to spot plugs through. Upon visual inspection, it was observed that the tubing is acceptable to spot plugs through, however I recommended that the tubing be pressure and drift tested to be sure it is clean and has no holes. Mr. Bobby Kroll stated he plans on having the tubing tested and steamed by the end of the week. Drilling-out of the cast iron bridge plug, which is set in the 4.5 casing at 2750, will commence this Friday.
