| The Department was at the above referenced Marcellus Well to conduct a Routine Inspection.  The operator is currently placing silt fence around the entire site because the silt sock was removed too early in a few places.  The These four wells are drilled and in production.  The site has obtained temp. stabilization.  The operator also contacted the Department about giving Atlas their access road back to their conventional well so they do not have to cross the Chevron Marcellus site every time they go to the well.  The Department reviewed the E/S Plan and approved the minor modification for the access road.  The modification is inside the original limits of disturbance and inside of the ESCGP-1 BMP's.  The minor modification was approved in the field and/or by email.

| The Department was at the above referenced well to conduct a routine inspection.  This inspection was conducted after a heavy rain event.  Grass is growing on site and considered temporary stabilized.  The Operator replaced the E/S Controls on site because they were removed before the site was permanently stabilized.  The site does have some E/S concerns.  The rain caused the fill slopes to start eroding and these will need re-graded before the problem gets worse.  The Department recommends waiting for good dry weather to conduct any earth disturbance activities.  The northern fill slope where the sediment trap was removed is also started to erode.  The erosion is mainly at the base of the slope.  The Operator may want to place temporary controls such as straw bales at the base of the northern slope to keep the sediment on site.  Please contact the Department at the phone number listed below if you have any questions.

2107389 2012-10-04 No Violations Noted The Department was at the above referenced Marcellus Well to conduct a Routine Inspection. The operator is currently placing silt fence around the entire site because the silt sock was removed too early in a few places. The These four wells are drilled and in production. The site has obtained temp. stabilization. The operator also contacted the Department about giving Atlas their access road back to their conventional well so they do not have to cross the Chevron Marcellus site every time they go to the well. The Department reviewed the E/S Plan and approved the minor modification for the access road. The modification is inside the original limits of disturbance and inside of the ESCGP-1 BMP's. The minor modification was approved in the field and/or by email.
2112684 2012-10-31 No Violations Noted The Department was at the above referenced well to conduct a routine inspection. This inspection was conducted after a heavy rain event. Grass is growing on site and considered temporary stabilized. The Operator replaced the E/S Controls on site because they were removed before the site was permanently stabilized. The site does have some E/S concerns. The rain caused the fill slopes to start eroding and these will need re-graded before the problem gets worse. The Department recommends waiting for good dry weather to conduct any earth disturbance activities. The northern fill slope where the sediment trap was removed is also started to erode. The erosion is mainly at the base of the slope. The Operator may want to place temporary controls such as straw bales at the base of the northern slope to keep the sediment on site.
