1917696 Primary Facility 2010-08-24 Routine/Complete Inspection No Violations Noted The Department was at the above Referenced Marcellus Well to conduct a Routine Inspection. The Operator is currently drilling a Marcellus Well on site. The site is in overall good condition. The E/S Controls are in good condition. The 32 inch silt sock appears to working as designed. The Department observed a contractor pumping water out of a ditch on the edge of the site. The Contractor said the fluid was fresh water. The Department used a meter to confirm they were pumping fresh water off the ground and that a release had not occurred.
2612095 Primary Facility 2017-06-12 Routine/Complete Inspection No Violations Noted On Monday, June 12, 2017, I conducted a routine-complete inspection at the NV25 Well Site located in Morris Township, Greene County.

The Act 9 sign was in place at the site entrance. The sign was legible but is slowly being covered with growing vegetation. The operator should consider maintaining the vegetation around the sign to assure its' visibility to potential emergency responders. The access road and the well pad are in good condition and the site is stabilized with vegetative growth.

The mailbox used to contain required documentation on-site was missing and later found crushed on the ground and buried in vegetation. The mailbox appeared to be void of any documentation. The ESCGP associated with site construction was terminated in January 2016. An error was made in a previous report written for NV20 where similar observations had been made. The NV 20 ESCGP was terminated in October of 2014 and not January 2016 as stated in the report. Although the ESCGP has been terminated, the operator shou
