2760113 Primary Facility 2018-08-01 Incident- Response to Accident or Event No Violations Noted On 07/31/18 Chevron notified the Department that heavy rains had caused BMP failures at the site.

On 08/01/18 Chevron provided the Department rain gauge data showing the site received 1.32 inches of rain between 1515 and 1530 hours.

On 08/01/18 at approximately 0900 hours I conducted an incident response inspection. Sediment basin #1 had received sediment, but still had remaining volume and appeared to be structurally sound. The diversion channel leading to the sediment basin had been over run and erosion was present on the north side of the channel. The erosion channel flowed to the basin and no impacts outside the LOD were observed.

Sediment detention basin #1 also received a large amount of sediment, but still had remaining volume and appeared to be structurally sound.

On the east side of the site sediment laden water was observed leaving the LOD between the topsoil stockpile and access road slope. Erosion was also observed within the LOD at this location. At the time of inspection Chevron was repair

2767278 Primary Facility 2018-08-23 Compliance Evaluation No Violations Noted On 08/23/18 at approximately 0845 hours I met with representatives of Chevron to discuss redline changes. Violations observed on 08/01/18 (inspection ID#2759849) had been corrected. Redline approval was granted for Chevron to add additional silt sock and hay bales and to use stacked silt sock to prevent further sediment discharges. The E&S plans were also redlined to reflect the installed position of a key drain near the site entrance.

There were no violations observed at the time of inspection.

2774113 Primary Facility 2018-09-11 Routine/Complete Inspection No Violations Noted The Act 9 sign was in place and all required documents were present at the site. Heavy rainfall had occurred in the area prior to inspection. Site BMPs were functioning at the time of inspection and there were no discharges off of site. The well pad was being brought to grade at the time of inspection. There were no violations observed at the time of inspection.
2796296 Primary Facility 2018-10-17 Preoperation Inspection No Violations Noted The Act 9 sign was in place and all required documents were present at the site. E & S BMPs were installed as designed and were functioning. Erosion matting, seed, and mulch were being applied at the time of inspection. A minor slip into the sediment basin was being repaired.

Redline approval was granted extending a cross culvert and adding a rip rap apron and shifting the drainage channel. Excess soil was stock piled off the south east corner of the pad, which prompted the extension of the cross culvert.

Chevron may commence drilling activities at their discretion.

2799818 Primary Facility 2018-11-07 Drilling/Alteration No Violations Noted Operator reports well spud 10/23/18. 40' of 30" csg set in a 36" hole with cmt poured down backside of dry hole. On site inspection shows no apparent violation.
2836996 Primary Facility 2019-02-07 Drilling/Alteration No Violations Noted The rig was shut down at a depth of 3199 for a crew training session at the time of this inspection. I conducted an entire site inspection with the DSM in charge and all equipment looked ok. The well permit was displayed and all log information was present and up to date.
