On 03/11/16 at approximately 0700 hours I conducted a routine complete inspection of the Zalac Unit. The Act 9 sign was in place. All required documents were present at the site.

The site was well vegetated and all BMPs appeared to be in good condition. Seven wells had been drilled and were protected by barriers. These wells were appropriately labeled. No E&S issues were observed at the time of inspection.

2515447 Primary Facility 2016-08-19 Routine/Complete Inspection No Violations Noted On 08/19/16 at approximately 1030 hours I conducted a routine complete inspection of the Zalac well site in response to a site restoration extension request submitted by Chevron. The Act 9 sign was in place and all required documents were present at the site. The site was well vegetated and no signs of erosion were observed. At the time of inspection the GPUs were being installed. The original date for site restoration was to be August 4th, 2016. A two year extension has been granted. The new date for site restoration will be August 4th, 2018.

2564597 Primary Facility 2017-02-24 Routine/Complete Inspection No Violations Noted The Act 9 sign was in place and all required documents were present at the site. A logging operation was in progress off the side of the access road and a log lay down area was adjacent to the access road. BMPs on site were properly maintained and appeared to be in good condition. Disturbed areas were present due to a pipeline operator installing a well connection pipeline. The disturbed areas were appropriately stabilized. The well site was well vegetated and no signs of erosion were observed. There were no violations observed at the time of inspection.
2636928 Primary Facility 2017-08-11 Routine/Complete Inspection No Violations Noted The Act 9 sign was in place and all required documents were present at the site. The purpose of the inspection was to meet with Chevron representatives to discuss redline changes to the site. Chevron requested redline approval to stockpile gravel on the well pad. The request was approved and a photograph documenting the change is attached.

The purpose of the inspection was to transfer previously approved redlines to a new set of plans. There were no violations observed at the time of inspection.
2652688 Primary Facility 2017-10-19 Routine/Partial Inspection No Violations Noted The Act 9 sign was in place and all required documents were present at the site. The purpose of the inspection was to discuss redline vegetation changes to basins 2 and 3. Due to the main areas of the basins holding less water than anticipated, Chevron requested Department approval to change to a seed mixture that would be more suitable for the site conditions. The requested change was approved.

The site was well vegetated and there were no signs of erosion observed. The production equipment appeared to be in good condition. There were no violations observed at the time of inspection.
2771353 Primary Facility 2018-09-06 Site Restoration No Violations Noted A site restoration inspection was conducted in response to an NOT submittal from Chevron. The Act 9 sign was in place and all required documents were present at the site. The site was well vegetated and met and exceeded uniform 70% vegetative cover. PCSM features were installed and appeared to be in good condition. The site meets the requirements for permit termination. The NOT is approved.
