On 06/01/16 at approximately 1400 hours I conducted a routine complete inspection of the Sickle Unit. The Act 9 sign was in place and all required documents were present at the mail box. The site was well vegetated and the site had been partially reclaimed. An area along the access road appeared to have been recently restored and graded. There were no violations observed at the time of inspection.

2768902 Primary Facility 2018-08-23 Routine/Partial Inspection No Violations Noted On 08/23/18 at approximately 1200 hours I met Curtis Small of Chevron to discuss transferring previously approved redlines to the PCSM page prior to Chevron submitting for an NOT. The Act 9 sign was in place and all required documents were present at the site. Redlines approved in 2014 were transferred to the PCSM sheet.

A small fluid release was observed on the site entrance outside the gated area. The fluid appeared to be similar in type to hydraulic fluid. A line of spilled fluid was observed leaving the site and continuing down the road. During the inspection agricultural equipment was observed traveling back and forth on the roadway adjacent to the site.

2809579 Primary Facility 2018-11-21 Site Restoration No Violations Noted A site restoration inspection was conducted in response to an NOT submittal from Chevron. The Act 9 sign was in place and all required documents were present at the site. The site was well vegetated and there were no signs of erosion observed. The site met and exceeded uniform 70% vegetative cover. PCSM features were installed and appeared to be functioning. The NOT is approved.
