2265501 Primary Facility 04/25/2014 Site Restoration Outstanding Violations - No Viols Req'd On 4/25/2014, I performed an inspection of Chevron Appalachia's Porter Naylor Unit 27H, 28H, 37H, 38H, 39H well site in Perry Twp., Fayette County. The reason for the inspection was that the operator is requesting an extension to the restoration date for the site.There are 5 wells on the site. One well is yet to be fracked. This is part of the reason for the extension requqest and the need to retain the water impoundment until that well is fracked. The impoundment had over two feet of freeboard and the water was being circulated today.

The Act 9 sign was in place and all required documents were present at the site. The site was being hydroseeded and BMPs appear to be in good condition and maintained. There were no violations noted at the time of inspection
2421489 Primary Facility 07/28/2015 Routine/Complete Inspection No Violations Noted On 07/28/15 at approximately 1500 hours I conducted a routine inspection of the site. The Act 9 sign was in place and all required documents were present at the site. The wells appeared to be in good condition. The site had been seeded approximately one month earlier. It was clear that the land owner had used heavy machinery to create a motocross type track on the site (see photos). The earth disturbance occurred in the lower section of the site and what appeared to be jumps were built over top of the reclaimed sediment trap.
2478946 Primary Facility 2016-04-25 Routine/Complete Inspection No Violations Noted On 04/25/16 at approximately 0800 hours I conducted a routine complete inspection of the site. The Act 9 sign was in place and all required documents were present in the mail box. The site contained disturbed areas that have been documented to be the result of the land owner causing third party impact Insp ID # (2421484). At the time of inspection the motocross type track was still present at the site (see photographs). The areas not impacted by a third party exhibited good vegetative cover (see photographs). There were no violations observed at the time of inspection.

2510395 Primary Facility 2016-06-17 Routine/Complete Inspection No Violations Noted

On 06/17/16 at approximately 1030 hours I conducted a CEI at the Porter Naylor site. The Act 9 sign was in place and all required documents were present at the site.

On 02/04/14 the Department conducted an inspection of the site and found violations of chapter 105 for failing to have the GP-7 permit present at the crossing and for failing to construct the stream crossing as approved. A violation of 102.8 was cited for failure to obtain PCSM for a paved apron at the access road entrance.

At the time of inspection the paved apron had been removed and the stream crossing had been reworked to be compliant with the approved GP-7. Chevron has corrected the violations cited in the 02/04/14 inspection report. There were no violations observed at the time of inspection.

2647818 Primary Facility 2017-09-28 Site Restoration No Violations Noted A site restoration inspection was conducted by this WQS and WQSS John Carson in response to an NOT submittal by Chevron. The Act 9 sign was in place and all documents were present at the site. Chevron had previously documented and provided evidence of third party impacts at the site. Third party impacts were observed at the time of inspection (see photographs). The remainder of the site not impacted by third party actors was well vegetated and met and exceeded 70% uniform vegetative cover. The site meets the requirements for permit termination. The NOT is approved.
