The site had an Act 9 ID sign up that contained the emergency contact numbers, the well names and permit numbers. The box locatd by the ID sign contained the E&S plan, PPC plan and well permits.

The access road was in good shape.

The area has been reclaimed and has good vegetative growth on it. Two sed traps are still on location. There are a few gullies forming on the hillside where the large impoundment has been reclaimed. The Operator may need to address those gullies in the Spring of 2014.

There are three wells on site and all where labeled. Three tanks are on site and are all located inside containment. There are three heater/separator units on site and the plumbing from the tanks is located above ground. No leaks were observed this day.

2357531 2015-04-01 No Violations Noted Well appeared to be equipped for production. Brine tanks located onsite and they appearedx to be in sound condition. There was no observed accelerated erosion or sedimentation in relation to the site or access road.
