We were called off the site due to an emergency but returned later in the afternoon and stopped at Chevron’s Hartley site.  Chevron was simultaneously filling a frack pond, vertical drilling a well and constructing the rest of the site.
 
 
There was an E&S plan in the tube in front of the site and a site sign.  We found several areas where the triple silt sock was over-run.  We also found bare areas where there were no E&S controls and areas sediment got off-site.  There were also bare slopes that did not appear stabilized within the prescribed 4 days.  These are all violations.
 
 
We left and I contacted Beth Smock about the problems.

On 06/24/16 at approximately 1300 hours I conducted a CEI for the Hartley Unit. The Act 9 sign was in place and all required documents were present at the site. Along the access road were small locations that were poorly vegetated (see photographs). It is recommended that these areas be addressed.

On 03/09/12 the Department received a complaint alleging run off from the site was impacting a privately owned pond. On 03/14/12 Department staff conducted an inspection of the site and found the violations listed below.

The purpose of the inspection was to meet with BA Lambie of Chevron to discuss vegetation on a steep slope at the site entrance. Due to the steep slope and what appeared to be animal activity small bare spots were present. After discussing the issue Chevron determined they would consider options of adding additional soil, seeding, and mulching.

On 06/24/16 at approximately 1300 hours I conducted a CEI for the Hartley Unit. The Act 9 sign was in place and all required documents were present at the site. Along the access road were small locations that were poorly vegetated (see photographs). It is recommended that these areas be addressed.

On 03/09/12 the Department received a complaint alleging run off from the site was impacting a privately owned pond. On 03/14/12 Department staff conducted an inspection of the site and found the violations listed below.

Violations of permit conditions 2, 7, 8, 18 25 Pa Code: 78.12, 102.11(a), 102.22(a)(1), 102.4(b), 78.53

A search of eFacts showed a record for 102.11, but the above violations were cited in the inspection report from 03/14/12.

At the time of inspection the site was stabilized with the exception of the aforementioned locations along the access road and there were no signs of erosion and sedimentation. There were no violations observed at the time of inspection.

2658251 Primary Facility 2017-11-02 Routine/Partial Inspection No Violations Noted The Act 9 sign was in place and all required documents were present at the site. The purpose of the inspection was to meet with BA Lambie of Chevron to discuss vegetation on a steep slope at the site entrance. Due to the steep slope and what appeared to be animal activity small bare spots were present. After discussing the issue Chevron determined they would consider options of adding additional soil, seeding, and mulching.
