2218003 | 2013-10-30 | No Violations Noted | On 10/30/2013, I conducted an inspection of the Chevron Appalachiaï¾’s Hamborsky 12 - 13H unit wells in Bullskin Twp., Fayette County.
The site hs a Act 9 sign up a the entrance of the site. The mailbox at the ID sign contained the E&S plan and well permits. The access road was in good shape and had a gate that was locked. Both wells were labeled and were hooked up to production. Two brine tanks are on site and are located inside secondary containment. The flow line to the two heater/separator units are underground. No leaks were observed. The site area has been reclaimed and has good growth on it except where the sed traps used to be located. The small gullies and wash outs observed in Septmeber have been removed, but there are areas of no growth in the old sed trap locations. The area of the old access road to the sed traps is also lacking the 70% uniform vegetative growth. The site looks to be stable. |

2223621 | 2013-11-20 | No Violations Noted | On 11/20/2013, the Department conducted an Notice of Termination inspection at the Chevron Appalachiaï¾’s Hamborsky 12 - 13H unit well site in Bullskin Twp., Fayette County. The NOT inspection is being done for ESCGP permit ESX09-051-0044.
The site had an Act 9 ID sign up at the entrance to the access road. The mailbox at the ID sign contained the E&S plan and well permits. The access road was in good shape and was gated and locked. There are two tanks on site located inside containment. There are two heater/separator units on site. Both wells were labeled. No leaks were observed today. The site has been reclaimed and all drilling equipment and supplies not needed for production have been removed. The vegetation on the reclaimed site looks good. The NOT is approved but it is suggested that the Operator check the vegetative growth in the Spring of 2014, in the area where the sed traps were reclaimed to be sure it continues to have the required 70% uniform coverage. |
