2012504 Primary Facility 2011-10-20 Routine/Complete Inspection No Violations Noted Conducted an on-site inspection to determine if proposed well site is within 100' of a blue line stream or a wetland greater than 1 acre, or pond greater than 1 acre in size. Upon inspection I found the well stake 187 feet from the edge of the nearest pond, that is greater than 1 acre in size. I spoke with Jeff Foley and Lori Nester of Enervest, and explained the options for moving forward. Lori indicated they would probably elect to keep the well pad site greater than 100 feet from the pond rather than apply for a waiver. I reported this to DEP permit reviewer, Amy Winger.
2023163 Primary Facility 2011-11-30 Routine/Complete Inspection No Violations Noted Upon inspection I found this new well pad site in the final stages of construction for the rig-setup. The rig was being delivered to the site while I was on-site. The permit and E&S Plan where posted and the BMPs implemented as per the plan. The access road was stoned and in good condition. The edge of the site was over 100' from the edge of (3) large ponds, with adequate BMPs in place to control runoff. No violations.

The permit was posted and the and E&S Plan was posted and implemented. The pits had greater than 2' of freeboard. The edge of the well pad was greater than 100' from the large ponds nearby. The access road was in good condition.

2034521 Primary Facility 2012-01-11 Routine/Complete Inspection No Violations Noted Upon inspection I found the rig had pulled out and well was plugged; the site was rough graded. The pits where closed. The BMPS remain in place, but the slit fence in the swale above the pond needs repair. Also additional BMPs are needed on the east side of the access road. I called Field Supervisor Jeff Foley and he said he would have the BMPs addressed right away.
2034680 Primary Facility 2012-01-17 Follow-up Inspection No Violations Noted Upon inspection I found the slit fence on the east side of the access road was down and sediment laden water was flowing towards the road ditch. Also a second slit fence barrier is needed in the swale between the pond and the well pad. I called Field Supervisor Jeff Foley and he said he would have the BMPs addressed right away.

Upon inspeciton I found an operator on-site removing the the stone from the ccess road. Some of the BMPs are been altered by the stone removal. Opeartor must ensure the BMPs are but back in place.
2038609 Primary Facility 2012-01-30 Follow-up Inspection No Violations Noted Follow-up to my 1/26/2012 inspection to determine if the BMPs had been repaired after removal of the access road stone. Upon inspection I found some of the BMPS had been repaired; however the slit fence located on the east side of the access road was down and in need of repair. Ground is currently snow covered and frozen with no runoff. I contacted Jeff Foley, Envervest Field Supervisor and he said they would get the BMPs fixed.
2041885 Primary Facility 2012-02-04 Follow-up Inspection No Violations Noted Follow-up inspecton to determine if the BMPs have been maintained as required. Upon inspection I found the slit fence at the north end of the site, in the low point that drains to the pond, has been undercut and is in need of repair. No sediment has entered the pond. It is recommended that a second string of slit fence be installed below the exsisting fence. All other BMPs are in place as required.
