| On August 26, 2016 at 1:00 pm I found what I believed to be the Winner 1 (035-20180) well, not labeled or tagged and about 300’ from the well location on file based on the original location plat. The exact location of the well is at 41.25170, -77.97974. The listed operator is “NEW YORK PETRO MIN CORP” and may be dissolved or non-existent at this point. I found the well off “Doc Lane” which started as a lease road off of Keating Mountain road and turned into a trail once I passed some water impoundments on the right. After going back in the woods a bit there’s a field with a cabin in the corner, and the well is in the field right off Doc Lane.The well was not operating at the time of inspection. All annuli were sealed and painted in uniform with the (7”) production casing (approx. 4’ above ground level) and wellhead fittings. Gauges were shut-in/isolated from well. There was not a production line out from the wellhead; all equipment had been broken down. The well and wellhead construction looked similar to a

| This report is to document that in reviewing the location plat of this well and others in the area as well as photos taken during my inspection end of last summer, it is clear the thought to be abandoned well is not the Winner 1 (035-20180), bur probably just an old pipeline riser. I spotted on an aerial image where I believe the Winner 1 well is located and will conduct a field inspection in the near future.
