The ESCGP that covers the PPHC B pad was part of Phase- 10 of the Punxsutawney Hunt Club (PHC) development. Phase-10 included the PHC-BB, PHC-CC, and PHC-DD Marcellus pads, and a PHC-AA that was never built. The PHC BB, CC, and DD pads have been fully restored to approximate original contour with an established perennial vegetative cover, and well site restoration reports have been approved for the wells on each of those pads. In order to allow the PPHC B pad to remain at full buildout size EOG Resources submitted a major modification to ESX11-033-0008 for installation of upgraded stormwater best management practices to the existing site. Stormwater BMPs have been installed per ESCGP modifications, the site is stable on cut and fill slopes, with a well established perennial vegetative cover, and compost silt sock has been removed. 2815155 Primary Facility 2018-12-12 Routine/Complete Inspection No Violations Noted A routine (pre-plugging) inspection of the PPHC 2H was conducted December 12, 2018. The PPHC 2H is a vertical, uncompleted, unconventional well.

I spoke with the plugging contractor on the access road and reviewed the tentative plugging procedure. On location, the 2H was inactive and capped. There cellar was partially filled with ice. There were three annular vents extending upward from the cellar. The vents were tee'd together and tied into a hose. The end of the hose was visually obstructed with snow/ice. There were no discharges, visible/audible venting, bubbling, or any other obvious issues observed at this time. A NOI to plug this well (2H) has been received. There were two other wells on the pad, the 3H in 4H, both in similar condition as the 2H. No obvious issues were observed at the 3H or 4H as well.

2819867 Primary Facility 2018-12-20 Plugging(Includes Plugged/Mined Through) No Violations Noted Plants and Goodwin are on location for final plug and abandonment of several vertical uncompleted wells. They have just finished plugging the PPHC 2H and are now rigged up on the PPHC 3H where they are running in the hole to attainable bottom. I met with the plugging contractor and discussed operations on the pad and plug depths. The 2H was uncovered, topped off with gravel with standing water on top. No bubbling or discharge was observed. No obvious issues were observed or reported at this time.
2824216 Primary Facility 2019-01-08 Routine/Complete Inspection No Violations Noted On January 8, 2018 I conducted an inspection of the PPHC 2H to investigate the operator's report of bubbling which has now developed approximately 3 weeks after plugging operations. At the time of inspection, the 2H was open at the wellhead flange, full of gravel with standing water on top. Steady bubbling was observed. Combustible gas was detected with an Altair 5x gas meter in the ambient air above the bubbling. I isolated an area that was bubbling with a plastic bag to confirm the presence of combustible gas; readings inside the bag quickly showed 100% LEL, 20% methane and increasing steadily. The plugging contractor is on location preparing to rig up to address the issue.

The 3H and 4H were open and visible; no obvious issues were observed at the 3H and 4H at this time.

2827277 Primary Facility 2019-01-11 Routine/Complete Inspection No Violations Noted On January 11, 2019 I conducted a routine (plugging) inspection of the PPHC 2H. A rig is on location, rigged up on the well at this time, preparing to run back in the hole to clean out in order to diagnose/address bubbling that developed after plugging operations. BOP has been installed and the crew is cleaning out a tank for returns. The 3H and 4H well cellars were open and full of ice. No new issues were observed at this time.
