Vegetation growing on disturbed areas of road at about 65% regrowth. Turnouts are becoming stabelized through vegetation. There are no E&S issues on first 200 ft of access road. While approaching well, a young bear came out of tree line and began following access road downhill. Due to implications this posed, my inspection ended at that point. Full inspection will be anticipated for morning of Friday, September 11, 2009

1829628 2009-09-11 Outstanding Violations - No Viols Req'd Inspection revealed the following:

-well tag is in place -ditches and previously eroded areas have been heavily lined with rocks. These areas appear to be stable -vegetation on slope above well has grown to about 30% -the slope with continuous erosional issues no longer has seed or mulch and multiple channels are beginning to form along slope -sediments laying against silt fence are deeper than what they were since last being noted -water and sediments have broke through sed fence and hay bales on south east corner of pad and off of southwest corner of "west virginia beach house" trailor where water has been directed to flow across road and into woodline. On-site meeting With Jim Lipsie will be held september 23, 2009 to discuss these issues

1830130 2009-09-23 Outstanding Violations - No Viols Req'd On-site meeting with Jim Lipsie of EAE, Contractor, Randy Lunger and one co-worker of Dan Beck Construction, and Well tender, Dan Stiver. Focused meeting on slope that continues to erode and can't sustain vegetation. Contractor proposed terracing the slope, spreading large amounts of lime and seeding with a high winter variety seed mix. BMPs will be cleaned up and replaced where necessary. Mr. Lipsie has to run suggestions through his boss and will get back to me as soon as possible with final plan. it has been suggested that the company have a soil test conducted, they use an engineer to design new contruction and create an effective inspection/maintenance schedule for BMPs.

-no changes have been made to date on this site. state of BMPs and slope remain as outlined in inspection reports from September 11 and 24, 2009.

-My Inspection revealed that the row of sliding materials and overburdened silt fence that was directly at the bottom of the hill has been pulled out. Soils were redistributed across slope and new fence installed along the contour. -about 1/3 of the way down the slope, an insloped terrace has been created. -the top edge of the slope has been bermed slightly to direct water into a catch basin rather than allowing it to flow across slope. -line of lime reached to about halfway down slope -disturbed area remains bare without stabilization -a rock lined sediment pond with water bar structure has been installed just prior to old trailor along road below well. -most of the old silt fence, with rotting haybales have been left in place

1843556 2009-11-05 No Violations Noted -Since last inspection all repairs to compromised BMPs have been made. Silt fence is installed and working properly, the water bar and erosion ditch that was formed in the curve in the lower end of the access road has been rock lined and sediment trap cleaned out.

-all disturbed areas that have not been addressed with rock lining are seeded and mulched with no signs of materials slipping downslope. -the addition of the terrace, upslope diversion ditch, water bar and sediment trap appear to be continually working to keep the slope from failing and reintroducing sediments into the nearby stream.

-slope has no apparant signs of failing. Dug into snow some to find vegetation has started growing prior to heavy snowfall. -silt fence shows no signs of failing or being overburdened. -Terracing and ditching of site appear to be remaining stable.
