2020140 2011-11-17 No Violations Noted At the time of inspection, well ID sign and Es plan present. Share access road stable. Rig onsite actively drilling top hole on 3h, 2h and 1H previously drilled yet not in production. Secondary containment installed beneath rig as well as tanks. Tanks located onsite appear structurally sound. Bmp's installed and functioning according to ES plan. Presviously disturbed areas appear to be seeded and muclhed with observed growth. Operator shall continue to monitor and maintain all bmp's until final/permanent restoration is achieved.

Noted that sediment trap located behind well site appears full--left Chevron Rep Curtis Small message regarding possible cleanout of sediment trap. As with last inspection, waters contained in sediment trap appear "black"- samples taken no anomalies found; therefore, discoloration believed to be result of decomposition of materials. No violations noted during this inspection.

Chevron representative was Ron Freed (Total Energy Services) and contracted Yost drilling. Conductor pipe (20﾿) was set at 40 feet and was grouted in place. First water surface stringer (20﾿) was set at 75 feet. This pipe was cemented by Superior with 125 sks of Class A and 1 bbls return to surface. Chevron was at 550 feet and encountered water bearing zone at 520 feet. Chevron was drilling to 600 feet and set the second water bearing stringer (13 3/8﾿ pipe).

Well granted inactive status 3/8/2012. 3H tophole drilled. 1H and 2H both drilled to TD. None yet frac'd. Well tag present. Tank on containment. Access road stable. No E&S issues observed although site snow-covered. One sed trap on-site. Department rep Samantha Foulk field-approved change to E&S plan. Upon completion of driling operations, topsoil will not be spread over the pad, the pad will remain rocked, and the pad size will not change.
