(24﾿ to 36 ft ﾿ sanded in) and 1st stringer (20﾿ to 80 ft ﾿ cemented with Type 1 cement 68 sacks with 7 bbl return to surface on 12/12/10.  According to Don Henson (chief), Jim Casselberry representing Highland gave approval for modification.  Dave Roberts (chief) made contact to Highland via email.  And they approved size of casing.  Also, the hole was extended to 670 ft.  with 18﾿ bit.  Rat hole was drilled.  Ziegenfuss rig will be moved off and Patterson Rig 61will be moved on to run 13 3/8﾿ and will cement to surface.  Drill cuttings placed in waste container and water to on-site tanks.

Small rig onsite. Access road stable. Bmp﾿s installed according to plan (ie. compost sox). No pits located onsite. Operator utilizing frac tanks, no IW issues observed. Noted placement of liner installed below rig. Recommend operator routinely maintain and ensure integrity of all bmp﾿s during drilling operations and winter/spring thaw as well as until final restoration is completed. Recommend that operator additional temporary bmp controls to surrounding soil pile to resist the potential for accelerated erosion. Accompanied by WQS Marc Maestra.

| On 1/11/11, dept received phone call and pictures from Matt Barch, of Chief demonstrating that roll off boxes containing cuttings have been removed. Based on pictures received, it was observed that some cuttings were still on unstable liner on pad, Mr. Barch assured me that this would be removed prior to rig moving back on site.  Then on 1/17/11, dept received preliminary written response (see attached).  An inspection was conducted on 1/20/11, site sign and Es plan onsite. Since last inspection, pad had been completely fenced in and 24hr security and emergency personal placed onsite. Rig currently setting up, pad covered with felt and then liner. Dept was unable to observe any remains of cuttings as all boxes and cuttings appear to have been moved off pad. No accelerated erosion or sedimentation issues noted. Access road stable. Accompanied by WQS Marc Maestra. Recommend that operator continue to closely monitor/maintain all bmps and temporary containment tanks.

Access road and well site adequately rocked and appear stable. No accelerated erosion and sedimentation issues noted. Temporary bmp﾿s installed and functioning according to plan, although observed that sediment trap reaching full capacity. Department recommended operator pump out sediment trap to prevent a possible pollution event in HQ watershed. Chief informed department that this site is currently under a DCNR Restriction of limited/no access, department has plans to contact DCNR for further information on restriction and possibility of allowing operator onsite to perform bmp maintenance and/or routine inspections as required by OG regs.
