1840930 2009-11-02 No Violations Noted Currently, site pad is in the process of completion. Inspected E&S plan and well sign located at entrance of site. Site design has been modified and initialized with changes. Filter fence and hay bails located along the perimeter of pad. Two pits dug at appropriate side wall ratio. Spud was called on 10/28/09 to the Williamsport DEP office. Conductor pipe has been set. Site has been seeded and straw covered surrounding the pad. According to PDC, rig will be starting on 11/16/09. Site located in high quality area. Well is going to be completed in the Marcellus. According to my conversation with Dave Hirko (Jackson twp manager), township was not notified of drilling event. DEP requested that PDC to contact township relative to ordinances (noise and work hours) and oil and gas township application. At the time of inspection, no E&S and IW issues. Adjacent properties have potable wells or springs as drinking water supplies.
1842252 2009-11-04 No Violations Noted At the time of inspection, the well was staked, not yet drilled. Contractors were actively excavating the well pad. The site was properly signed with E&S plan available for review. I noted that the site was in an HQ watershed; therefore, regulation requires the installation of increased/additional BMP¿s. A message was left with Jeff Clark on 11/10/09 discussing the additional BMP¿s. I observed the installation of filter fabric fence and culvert ditches. I am recommending that compost filter socks be utilized and/or additional BMP¿s with the filter fabric fence if it is not replaced. In addition, it is recommended that riprap be added to the culvert outlets/ditches until seeded vegetation produces growth. Disturbed areas and topsoil piles were temporarily seeded. Pits were constructed, but not yet lined. I am unsure of disposal method for drill cuttings; I plan to discuss this with PDC. No accelerated E&S issues were noted in relation to site or access road.
1844192 2009-11-16 No Violations Noted Currently, conduct pipe set at 40¿. 20 mil plastic aligned drilling pit and frac pit aligned also and security fence. Additional gravel added to entrance way. Township is concern with entrance way with gravel. Rig at 100 ft. First string is going to be set at for 1186¿ 13 3/8¿. At the time of inspection, no E&S and IW issues. Adjacent properties have potable wells or springs as drinking water supplies. Started drilling on 11/15/09
1845220 2009-11-20 No Violations Noted Currently, conduct pipe set at 40¿. 20 mil plastic aligned drilling pit and frac pit aligned also and security fence. No free board issues with pit. Additional fine gravel added to entrance way. Rig depth at 687 ft. First string is going to be set at 636 ft - 13 3/8¿ using 607 sks. With 60 bbl return. At the time of inspection, no E&S and IW issues. Barrier socks were set on down gradient of site pad. Filter fence and hay bails are also present.
1845498 2009-11-24 No Violations Noted Currently, conduct pipe set at 40¿. 20 mil plastic aligned drilling pit and frac pit aligned also and security fence. No free board issues with pit. Additional fine gravel added to entrance way. Rig progress at depth of 4385 ft. First string is going to be set at 636 ft - 13 3/8¿ using 607 sks. with 60 bbl return. Second string set at 1486 ft ¿ 8 5/8¿ pipe using 424 sks with 18 bbl returns. Small gas hits at 3892¿, 4052¿ and 4187¿. At the time of inspection, no E&S and IW issues. Rig shut down from 11/25/09 to 12/2/09 expected additional 5 to 7 working days of drilling.
1847634 2009-12-03 No Violations Noted At the time of inspection, the well was actively being drilled and site was properly signed with E&S plan available for review. Noted that both pits were lined and 2ft of freeboard was being maintained. I also noted the installation of compost sock down gradient form the site as well as filter fence reinforced with hay bales. No accelerated erosion or sedimentation issues were noted in relation to well site or access road. Access road was adequately rocked and also noted the addition of rock to culvert in/outlets. Recommend that operator continue to monitor access road and township roadways for mud and when necessary have a broom truck onsite to maintain roadways. I received a message from Ed Gahagn of PDC on 12/04/09 that he had plans to get a broom truck onsite to remove all mud/debris from township roadways. While onsite I talked to Miles Pierson (MPG Drilling), who noted the fact that drilling operations were commenced from 11/25/09 and returned on 12/2/09 for the holiday and they were expecting to be onsite several more days.
1847954 2009-12-07 No Violations Noted At the time of inspection, no E&S and IW issues. Pit aligned with 20 mil plastic aligned drilling pit and frac pit aligned also and security fence. No free board issues with pit. According to PDC, rig progress at depth of 8326 ft and anticipated depth should reached later part of day and followed by geophysical logging. At the time of inspection, no E&S and IW issues. Hydro frac of well has not been scheduled
1851675 2009-12-22 No Violations Noted At the time of inspection, no E&S and IW issues. Pit aligned with 20 mil plastic aligned drilling pit and frac pit aligned also and security fence. No free board issues with pit. According to PDC, rig will be shut down and removed from site due to problems with drilling operation with last 1500 ft of well. PDC will restart drilling operation in spring of 2010. McCutchen on-site to remove drilling fluids and remaining mud from site for disposal at facility. At the time of inspection, no E&S and IW issues.
1864822 2010-02-22 No Violations Noted At the time of inspection, no E&S and IW issues. Pit aligned with 20 mil plastic aligned drilling pit and frac pit aligned also and security fence. No free board issues with pit. Site is in the process of set up at 7900 ft and drill the last 1200 ft. According to Vaughn Staples (VBS), < 1 gallon of hydraulic fluid leak from drilling part. Spill was contained and absorbent pads were used along with hand digging of contaminant soil. Pads and soil were contained in drum. Additional pads were used around drilling part. At the time of inspection, no E&S and IW issues.

1896851 2010-07-08 No Violations Noted At the time of inspection, no E&S and IW issues. Site has been restored. Access road has been moved according to revised E&S plan. Well head consist of 13 x 8 x 4 x 2 inch piping with multiple shut offs. No signs of gas leakage around well head. Pressure at head consists of 620 psi casing, tubing 576 psi and 3306 MCF. Production equipment consists of tower, separator, compressor and 200 bbl tank (< 20 bbl fluid).
1900142 2010-07-21 No Violations Noted This is an ESCGP-1 site. At the time of inspection, well was drilled, properly signed, and in production. ESCGP-1, PPC, and E&S plan onsite. Operator notified department on 06/15/10 landing farming was to begin 06/18/10. Pits were successfully removed and site was regarded and seeded and mulched. Growth was observed, but not quite 70% uniform coverage. It is recommended that operate reseed some areas on pad that do not demonstrate successful growth in order to achieve uniform vegetative coverage. Brine tank surround by dike is stable. Access road was relocated around lower side of pad (disturbance area was not increased). Access road was stable, but over time it is recommended that operator add additional rock to prevent any erosion or sedimentation concerns that may arise. Operator should continue to maintain culvert outlets. Upon final restoration, filter fence may be removed. (a future inspection will be conducted to ensure adequate uniform coverage of previously disturbed areas). No IW issues were noted.
1915914 2010-09-22 No Violations Noted ESCGP-1 site. At the time of inspection, well was drilled, properly signed, and in production. Drilling completed on 02/28/09. ESCGP-1, PPC, and E&S plan onsite. Pits were successfully removed (land farming of cuttings) and site was regarded and seeded and mulched with successful, uniform growth. Brine tank surround by dike appears sound, although noted discoloration along side of tank, but no dead vegetation was observed below or surrounding tank. Access road is relocated around lower side of pad (disturbance area was not increased). Access road appears stable, with adequately rocked culvert outlets. Operator should continue to maintain culvert outlets. Recommend that operator remove filter fence, as it appears site has be adequately and uniformly permanently restored. Also recommending that operator relocate and/or secure ALL chemicals onsite from third party access. No accelerated erosion or sedimentation issues were note din relation to well site or access road. Accompanied by WQS Dana Burk during inspection.
