No coal seams or methane detected nor any voids were detected during the inspection. Pit aligned with 20 mil plastic and no free board issues. Spud date called on 9/29/09. TD of well is 9250 ft depth and drilled to Marcellus and vertical well. During the inspection, pit showed no signs of freeboard and industrial discharge. At the time of the inspection, no E&S issues.

No coal seams or methane detected nor any voids were detected during the inspection. Pit aligned with 20 mil plastic and no free board issues. Chesapeake was projecting to set 9 5/8¿ at 1550¿ during the weekend. During the inspection, pit showed no signs of freeboard and industrial discharge. At the time of the inspection, no E&S issues.
1845451 2009-11-18 Violation(s) Noted At the time of inspection, the well was actively being drilled. Site was properly signed with E&S plan onsite for review. Access road was in good condition, no accelerated erosion or sedimentation issues were observed in relation to well site or access road. Compost filter socks were properly installed and functioning. Drill pit freeboard of 2ft had been exceeded. Spoke with drilling operator onsite and noted they had halted drilling and made plans to remove/haul drill fluids immediately in order to sustain freeboard. It is recommended that the operator condition to closely monitor the 2ft freeboard and also install a baffle on the drill pipe leading to the pit in order to aid in preventing the above violation. 576371

Chesapeake set first string - 9 5/8¿ at 1487¿ with 262 sks and no return but cement was visible from surface. Project will continue through week and projected to possible complete during the week of 12/1/09.

Noma Drilling Rig 8 was at 9742 ft depth deeper than anticipated depth of 9587¿. Depth of shale formation is deeper than anticipated but did not enter into the Onondaga Ls (conservative well). Noma set new depth was 9855¿. Permit was on-site. Well log was present at the site. Stratigraphy consisted of sandstone and shale in this section. Pit aligned with 20 mil plastic and no free board issues.
1864625 2010-02-22 No Violations Noted Well site has been drilled. Service rig was on-site at time of inspection. There is no evidence of accelerated erosion or sedimentation on the access road or well site. Pit appears to be in sound condition. Due to weather conditions, when the snow begins to melt the operator should closely monitor the site for erosion and sedimentation.

1918465 2010-10-07 No Violations Noted At the time of the inspection, no E&S issues and no IW issues at the site. Well sign and E&S plan was located at the entrance during was present. Hydrocarbon Drilling has been contracted to plug well. Rig was set up over well and flow line was installed to pit. Pit aligned with 20 mil plastic and with partial security fence. Hydrocarbon has release pressure off well from 2950 psi to 1100 psi. Attempting to kill well and set plug. During the inspection, pit showed no signs of freeboard and industrial discharge.
1920003 2010-10-12 No Violations Noted drilled (12/07/09) and actively being plugged. Onsite with Jim Johnson and Kevin Swiger of Chesapeake Energy to review final site restoration plans. Operator intends to remove all pit contents (liquid, cuttings, and liner) and transport to an approved land fill. Depth and operator observe that pit contents are orange and level of pit appears to be slightly down, recommended that pit is addressed immediately by first removing liquid fraction. No accelerated ES issues observed, site is relatively flat, formerly farm fields. Operator plans to have pits removed, and site re-graded and seeded/mulched within next several weeks to preexisting conditions. Operator was also notified that if optimal growth (70% uniform veg. cover) is not achieved prior to first frost/snow, reseeding and mulching may be required in the spring in order to achieve permanent final restoration, and at that time all temporary bmp¿s may be removed. 1920832 2010-10-19 No Violations Noted At the time of the inspection, no E&S issues and no IW issues at the site. Well sign and E&S plan was located at the entrance during was present. Hydrocarbon Drilling has been contracted to plug well. Rig was set up over well and flow line was installed to pit. Pit covered with 20 mil plastic and with partial security fence. Hydrocarbon has set bottom plug at 9413 ft. Set 100 ft of cement on top of plug. Tried to shoot 5 ½ ¿casing at 4976 ft and 4230 ft. but could not pull casing with 115,000 psi to 160,000 psi. Confirmed no gas zones behind pipe in the shallow Devonian zones with elog and drillers log. Moved up to 1700 ft and cut the 5 1/2 ¿ and was going to set an upper bridge plug and add 250 ft of cement plug on top of the plug. Since it was coal permit, remaining will be left open with monument on surface. During the inspection, pit showed no signs of freeboard and industrial discharge.
1945532 2011-01-10 No Violations Noted During this inspection, well plugged with erected monument with attached API #. Site properly signed. Site snow covered, appears that pits have been removed and site restoration begun, although 70% successful vegetative cover could not be confirmed due to snow cover. Temporary bmp¿s installed surrounding site. No accelerated erosion or sedimentation issues noted, but observe that manure/hay had been recently spread over site. A follow up inspection will be conducted in spring to ensure final restoration is achieved; if <70% vegetative cover observed operator may need to reseed and mulch site, until then operator should continue to maintain all temporary bmp¿s Access road ok.
