|  There are no signs of erosion and sedimentation on the access road and well site. Brine tank is in sound condition. However, there is a spring popping up on the bottom slope of the sight that appears to have the characteristics of acid mine drainage. The AMD is making its way into an unnamed tributary to beaverdam run, having undercut the silt fence. The hillside seepage is orange in color, has slight sulfur smell, and a sheen in stagnant areas. A catch basin large enough to accommodate the volume of water should be installed at the lowest point of the site. The basin should be lined with limestone to help mitigate the AMD.  In addition the outlet of the basin should be rock lined to minimize erosion and sedimentation. The vegetation is adequate in the areas not affected by the AMD.

| Performed a routine inspection to address the erosion issue on site. Met with Vernon Fulmer on site to discuss proper BMP's to be implimented in order to prevent erosion and sedimentation.  The recommended BMP discussed was as follows: remove sediment that has accumulated at the bottom of site create permenant sediment trap lined with rip rap to prevent erosion and sedimentation. Tank sound. No signs of erosion on access road. The operator should continue to monitor the access road and well site for erosion and sedimentation.

| Performed routine inspection to evaluate the work performed to address the erosion issues above and below the access road. The work performed was determined to adequately address the erosion issues at the site. The access road coming down to the site is starting to show signs or erosion due to the heavy rains as of late. The operator should continue to monitor the access road and well site for erosion and sedimentation.

Inspection Data

Inspections Performed

Inspection ID Inspection Category Inspection Date Inspection Type Inspection Result Inspection Comments
1806975 Primary Facility 2009-06-23 Site Restoration No Violations Noted There are no signs of erosion and sedimentation on the access road and well site.
1815459 Primary Facility 2009-07-24 Routine/Complete Inspection No Violations Noted Performed a routine inspection to address the erosion issue on site.
1817861 Primary Facility 2009-08-03 Routine/Complete Inspection No Violations Noted Performed routine inspection to evaluate the work performed to address the erosion issues above and below the access road. The work performed was determined to adequately address the erosion issues at the site. The access road coming down to the site is starting to show signs or erosion due to the heavy rains as of late. The operator should continue to monitor the access road and well site for erosion and sedimentation.
