| In addition to the use of the Brandt centrifuges to separate solid and liquid material in the drill cuttings, chemicals used for coagulation and flocculation are injected into a pump prior to the centrifuge process.  Calcium nitrate and a polymer are placed into a mix tank housed in a dewatering trailer to go through a slurry feed process.  A request for an approval of alternative waste management practice (OG-71) needs to be submitted to the Department in relation to the drill cuttings processing and drilling mud dewatering at this location.  The request for an OG-71 approval encompasses both the use of centrifuges for separation of solid and liquid material in the drill cuttings during water based and oil based mud drilling  and the injection of the chemicals mentioned above used for coagulation and flocculation during water based mud drilling.

Small pieces of foam berm material were scattered around the pad and there were small pieces of felt in many locations. On the south side there was a large pile of frac sand. Most of the puddles on location gave field conductivity readings > 3 mS/cm. A 15' x 6' puddle in front of the sand pile gave field conductivity readings > 8mS/cm (sample 2371 381). Small, (~2' diameter) fuel stains were also found on E &W sides. The result of this inspection for the 3H well (015-22732) is pending the sample results.

Calcium nitrate and a polymer are placed into a mix tank housed in a dewatering trailer to go through a slurry feed process. A request for an approval of alternative waste management practice (OG-71) needs to be submitted to the Department in relation to the drill cuttings processing and drilling mud dewatering at this location. The request for an OG-71 approval encompasses both the use of centrifuges for separation of solid and liquid material in the drill cuttings during water based and oil based mud drilling and the injection of the chemicals mentioned above used for coagulation and flocculation during water based mud drilling.

2273143 Primary Facility 2014-05-20 Routine/Complete Inspection Pending Arrived 15:00. Workers painting well head. Groups of yellow baker tanks were stationed on the S and W sides of the pad and in the NE corner. Small pieces of foam berm material were scattered around the pad and there were small pieces of felt in many locations. On the south side there was a large pile of frac sand. Most of the puddles on location gave field conductivity readings > 3 mS/cm. A 15' x 6' puddle in front of the sand pile gave field conductivity readings > 8mS/cm (sample 2371 381). Small, (~2' diameter) fuel stains were also found on E &W sides. The result of this inspection for the 3H well (015-22732) is pending the sample results.

2519144 Primary Facility 2016-09-15 Administrative/File Review No Violations Noted Administrative inspection conducted in repsonse to well site restoration reports (WSRR) filed by Chief on 09/09/2016. The notice of termination for this site was previously approved by the Department on 07/22/2016 (Inspection No. 2506949).

Interim restoration has been implemented on the well pad. A significant portion of the pad surface has been decompacted and vegetated. A total of 5,160 tons of drill cuttings, 711 barrels of drilling fluid, and 21,914 barrels fracing fluid are reported to have been disposed of at appropriate waste disposal facilities.

The well site restoration reports are acknowledged.

2814349 Primary Facility 2018-12-12 Routine/Complete Inspection No Violations Noted Arrived onsite for a routine inspection at 09:57; the weather was ~30°F and calm. There was a light dusting of snow on the pad and surrounding area.

There were 3 tagged Marcellus wells staged for production. The 3 well cellars were filled with gravel. The 2H well tag shows Stevens Twp., however, the permit sign near the mailbox shows Wilmot Twp. (Photos #2, 3).

There were 3 GPUs, 4 produced water tanks, and a compressor onsite and in secondary containment. There was also a freshwater injection tank plumbed to the wells.

The pad area was reduced and the site was greater than 70% vegetative cover. Both the cut and fill side of the pad had steeper sections that were stabilized with rock. The site appeared stable; no E&S issues were noted.

It is recommended that the tags for the 2H well be corrected as needed.

No violations were observed during the inspection.
