2707347 Primary Facility 2018-03-20 Drilling/Alteration No Violations Noted This well has a 6' cellar. The well was spudded to 40' with a 36" bit. The operator ran 40' of 30" conductor pipe and cemented it in using 5.5 yards of class A cement. There was no water encountered in the cellar. The cellar is covered over with a piece of steel plate for safety until the top hole drilling rig arrives to drill. All required paperwork is on site and the equipment on site looks ok.
2707392 Primary Facility 2018-03-20 Routine/Partial Inspection No Violations Noted Inspection conducted in response to spud notifications for wells 3H, 4H, and 5H.

At time of inspection, well 3H drilled to 40 feet and covered with steel plate. Wells 4H and 5H drilled to 40 feet and covered with steel plates. Wells 1H and 2H drilled and frac'd.

Department unable to adequately inspect the entire location due to ongoing high pressure testing occurring on part of the well pad.

2732749 Primary Facility 2018-05-22 Drilling/Alteration No Violations Noted This well has a 6' cellar. The well was spudded to 40' with a 36" bit. The operator ran 40' of 30" conductor pipe and cemented it in using 5.5 yards of class A cement. There was no water encountered in the cellar. The rig ran 152' of 20", cemented with 337 sks of cement and saw a 7 bbl return on cement. Drilled to 890', ran 888' of 13 3/8" pipe, cemented it with 691 sks of cement and saw a 31 bbl return on cement. The Rig had drilled to 2006', ran 1986' of 9 5/8" casing and was getting ready to cement at the time of this inspection. The drilling log information and all equipment on site looked ok.

2823170 Primary Facility 2019-01-08 Routine/Complete Inspection No Violations Noted This well is tagged, in production and temporarily shut in for a sales line upgrade which is taking place at the time of this inspection in order to provide additional pipeline capacity for the 5 wells on this unconventional well pad. The well head casing, piping, production set equipment, containment dike and tank battery look ok.
2828387 Primary Facility 2019-01-17 Routine/Complete Inspection No Violations Noted On 1/17/19 at 0903 hrs., I conducted a routine inspection of Snyder Brothers, Inc.'s Kuhns well pad and associated wells in Boggs Twp., Armstrong Co. Five wells have been drilled and are equipped for production, including well 3H (API 005-31302). Well tags are present at the well heads. There are no violations noted for this inspection.
2853263 Primary Facility 2019-03-05 Routine/Partial Inspection No Violations Noted At the time of inspection, well was producing. Well integrity was in good condition, with no signs of any leaks or spills onsite. The well did not have any corrosion. The cellar was below ground level, with water inside. No bubbles were seen. The digital gauges on the well showed the tubing pressure to be 1707 psi and the casing pressure was 2015 psi. The well was properly labeled and the Emergency Response sign was posted at the site entrance. There were 3 dual line heaters and 5 separators onsite. The 1H & 2H wells had smaller separators than the rest of the wells. There were also 3 tall tanks and a smaller horizontal tank contained within a lined, steel dike.
