| The permit 005-30785 expired on 02/11/13.  Well-site and access road had been constructed except for a stream crossing of an Unamed Tributary (UNT) to Brady Run.  This stream crossing would have required a General Permit.  The access road as constructed had minimal to no implemented  erosion controls, especially on the steepest portion of the road as it approached the UNT from the main access road for the project area.  In addition the access road had no erosion controls implemented for the portion of the road across the UNT approaching the well pad.  The well pad itself had not been temporarily stabilized which resulted in sediment generated and transported onsite eventually overtopping the silt fencing implemented.
