1959591 Primary Facility 03/16/2011 Routine/Complete Inspection Violation(s) Noted Well is drilled, but not equipped for production. Pit is still on-site. Several Violations were noted-
1997597 Primary Facility 08/22/2011 Routine/Complete Inspection Outstanding Violations - No Viols Req'd Drilling was completed December of 2010. Well appeared to be equipped for production. It did not appear that the issues outlined in WQS Shultz¿s inspection regarding erosion controls had been addressed. Site had been regraded, however the vegetative cover did not meet the 70% uniform coverage criteria. Site had no erosion controls to isolate the disturbed area for the site from the unnamed tributary (UNT) of Glade Run. Evidence was observed of sediment transport via this UNT and on the access road.
