1997597 2011-08-22 Outstanding Violations - No Viols Req'd Drilling was completed December of 2010. Well appeared to be equipped for production. It did not appear that the issues outlined in WQS Shultz﾿s inspection regarding erosion controls had been addressed. Site had been regraded, however the vegetative cover did not meet the 70% uniform coverage criteria. Site had no erosion controls to isolate the disturbed area for the site from the unnamed tributary (UNT) of Glade Run. Evidence was observed of sediment transport via this UNT and on the access road.

2091024 2012-08-09 No Violations Noted At the time of inspection the well appeared to drilled and equipped for production. The associated well apparatus located onsite appear to be in sound condition. No observation of accelerated erosion or sedimentation in relation to the well pad or access road. The site appears to be restored and meets the 70% uniform vegetative coverage criteria.
