| Drilling was ongoing at the time of the inspection.  Below grade pit located onsite at the timem of the inspection and it had more than the required two feet of lined freeboard.  Observed no accelerated erosion & sedimentation in relation to the access road or well-site.  Silt fencing was installed properly.  The Department recommends that the operator seed & mulch the disturbed areas along the access road provided they will not be further disturbed during well development activities.  It was also noted that in the E&S plan the soils information indicated that the seasonal high groundwater table rises to within 6-18 inches of the surface, however during the site inspection no evidence was found to support the seasonal high groundwater table being at that shallow depth.

1877864 Primary Facility 2010-04-19 Routine/Complete Inspection No Violations Noted Drilling was ongoing at the time of the inspection. Below grade pit located onsite at the timem of the inspection and it had more than the required two feet of lined freeboard. Observed no accelerated erosion & sedimentation in relation to the access road or well-site. Silt fencing was installed properly. The Department recommends that the operator seed & mulch the disturbed areas along the access road provided they will not be further disturbed during well development activities. It was also noted that in the E&S plan the soils information indicated that the seasonal high groundwater table rises to within 6-18 inches of the surface, however during the site inspection no evidence was found to support the seasonal high groundwater table being at that shallow depth.
1898179 Primary Facility 2010-07-14 Site Restoration No Violations Noted Drilling had been completed, well appeared to be equipped for production at the time of the inspection. Brine tank located onsite and appeared to be in sound condition. Site had been regraded, seeded, and mulched. Vegetation had begun to grown but did not as of yet meet the 70% uniform coverage criteria. The operator should conintue to monitor the site for this and if neccesary reseed and mulch the site. No accelerated erosion & sedimentation was observed in relation to the access road or site. Noted that the entire access road had been finished with clean durable gravel.
1964073 Primary Facility 2011-04-07 Site Restoration No Violations Noted Drilling completed on 04/20/10. Well appeared to be equipped for production at the time of the inspection. Brine tank located onsite and appeared to be in sound condition. Observed no erosion & sedimentation in relation to the access road or well-site. Vegetative cover had not as of yet achieved the 70% uniform coverage criteria. The operator should reseed and mulch the site this growing season to achieve the 70% coverage criteria. Once this achieved the operator should remove the silt fencing erosion control BMP.
2215215 Primary Facility 2013-10-18 Routine/Complete Inspection No Violations Noted Well appeared to be equipped for production. Brine tank located onsite and it appeared to be in sound condition. There was no observed accelerated erosion or sedimentation in relation to the site or access road.
2466991 Primary Facility 2016-03-22 Routine/Complete Inspection No Violations Noted Well tagged-in production. Annulus is open. Well head casing-equipment-100 bbl tank-dike look ok.
