-1853860812 2012-03-27 My inspection this day is routine - I found well equipped as gas well with all valves closed. The tank nor well didn't have identification and there was a very old paper chart in meter with no information. They will need more dike to meet SPCC requirements. Well needs to be placed back in production, plugged or placed in TI status. 9-9-05(A)10

-1959659762 2013-12-11 There has been no attempt to gain compliance since the last inspection conducted by inspector Cindy VanDyke on 3/27/12. Well is still set up with a rabbit catcher and there is a black plastic line that runs to the house that is closest to the well. No pollution or contamination observed. Production records show that the well has not produced for the past two years. I will contact the company to remind them of the existing compliance notice.
-691836879 2014-01-06 Spoke with Jordan from G-R contracting on the phone he stated that this and the other Stark well were on his to be serviced. He stated that he knew he would be on them within 6 mos. He is also aware that that one of the homeowners is receiving house gas.
1704365595 2014-06-06 Upon inspection, it was observed that this well is currently shut in, but appears to be capable of production. I contacted Jordan from G-R Contracting and he stated that their rigs will be mobilizing from Monroe County to Washington County by the end of June and they plan on reworking this well. G-R's pumper for the area, Mark Foreman, met with me on the well location and installed new identification information at the well head and the common tank battery. This satisfies the requirements of the compliance notice.
1711697400 2005-06-13 Inspection this day found well capable of producing except does not have chart in meter, valves open and believe landowner getting house gas - at least from one of the wells here - tank has pealing ID's, fair dike. Will continue to monitor as believe not selling much from this well.
