1078754878 2004-03-08 Production Wells Violations Noted Inspection conducted at the request of Mr. McCandlish phone call of 03/05/04. His gas being out because Mr. Seifert blowing gas off into the atmosphere which he knows is illegal. Inspection this date revealed Mr. McCandlish had shut the valve that was allowing the gas to be vented, and he now has gas. I did note a violation at the location for illegible identification. Violation of 1501:9-9-05 (A) (10) O.A.C. 03/17/04 NOV returned to Bellefontaine office and shown NOT DELIVERABLE AS ADDRESSED - UNABLE TO FORWARD.

1655435735 2012-05-10 Annular Disposal No Violations The Division received a call from landowner alleging idle wells on property. The landowner does not want to file a complaint at this time. A follow-up inspection conducted with Kip Thompson revealed that a service rig has been placed on the well and setup to swab into tanks. The tank battery location consisted of a (2) 100 bbl with a standing knockout tank and no earthen dike. The knockout tank contained a disconnected line (plugged with paraffin) with a steady drip (approx 1 drip per second) from nipple onto ground surface. The fluid was tested with High Range Quantabs and showed maximum of 10 units on titrator. Note: 8.8 units equals 6562 ppm (mg/L) Cl-. Notified the well owner, Mr. John Seifert, while on location. Mr. Seifert arrived at location within 15 minutes and notified employees of repair. Prior to leaving the location, his crew was repairing the leak. Mr. Seifert plans to place well back into production therefore, was instructed to build an earthen dike, provide identification at the tank; and cleanup around the tank battery. Note: The tank location located next to saw mill will most likely be moved the future to allow for more room to operate and also serve as a shared tank battery with existing wells on property.
2062052196 2005-05-18 Production Wells No Violations Inspection revealed well is in operation. Inspection of RBDMS revealed no production records have been filed since 1997 for this well.
2135510707 2019-01-18 Production Wells No Violations I reveived a request from central office UIC section to inspect this well to determine if the well is capable of production. The well is capable of production. The well consists of a pupmping unit. The tank battery consisted of 2-100 barrel tanks.
