1306255770 2011-05-24 I performed a rig inspection on Wildcat rig #1. The drilling depth was 7418' and drilling on fluid. The zone that they were in was the Rose Run. This inspection satisfies 1501:9-9-03. There were no violations at this time.
1307549516 2011-06-06 The drilling and blow pits fluid level were down 4 feet from the surface. The rig is gone and debris hauled away. There were no violations at this time.
1309482463 2011-06-23 On May 25, Universal completed a two stage cement job on this well. The primary cement job successfully covered the Rose Run to Black River interval as designed. The second stage encountered circulation problems which raised concerns about the cement coverage. On June 23, Phoenix Well Services ran a cement bond log which indicated that the second stage cementing only covered the DV tool set at 4820' (near the top of the Queenston) to the bottom of the Packer Shell (4570'). Phoenix perforated the 4-1/2 casing with 8 shots from 4568' to 4570'. Universal broke circulation at 1400 psi and pumped 30 bbl of fresh water ahead which lowered the pressure to 570 psi. Universal then mixed and pumped 500 gallons of Unisweep, 600# of calcium chloride and 800# of Flo-stop P before mixing and displacing 350 sacks of 50/50 pozmix with 2% gel, 10% salt and 0.75# TI-2. Circulation was maintained throughout the job. Displace to 4518' with plug down at 12:27 pm on 6/23/2011.
-1654647507 2011-06-09 I met with Tom Wood, Ray Pander and RC Pander at the drilling location to witness a the running of the bond log. The logging company was Phoenix Well Services from Delmont PA. The top of the cement was 4580' which is at the base of the Packer Shell. There were no violations at this time.
-1656830425 2011-05-16 Chris Grimm, John Brauer and myself preformed a rig inspection. The drilling rig, Wildcat rig #1 has arrived on the drilling location. The drilling pits have not been dug yet. There were no violations at this time.
-390705618 2011-05-17 Chris Grimm, Ray Wallace and myself preformed a rig inspection on Wildcat rig #1. The rig crew were circulating the 11 3/4 conductor pipe. This inspection satisfies 1501:9-9-03. There were no violations at this time.
665420439 2011-05-19 I performed a rig inspection on Wildcat rig #1. The drilling depth was 1848' and drilling on air in the shale. I wavied witnessing the BOP test. The BOP was tested to 575psi and lost 8psi in 20 minutes. This inspection satisfies 1501:9-9-03. There were no violations at this time.
-674688489 2011-05-18 I preformed a rig inspection on Wildcat rig #1. The rig crew were running into the hole with 8 5/8 casing. This inspection satisfies 1501:9-9-03. There were no violations at this time.
-705104514 2011-06-30 Chris Grimm and myself witnessed the bond log ran by Phoenix Well Services. Tom Wood and Ray Pander were also present. There were no violations at this time.
-744386438 2011-12-13 All drilling pits are closed. One top soil pile has been spread on the drilling location. There were no violations at this time.
-867757163 2011-08-02 The drilling pits were closed. The workover has not started. I went to Pander's office and spoke with RC about well status. He stated that he is talking with Tom Tomasick about opening up the Newburg. Tom wanted more information on the Newburg formation. There were no violations at this time.
