-1268898425 2013-10-09 I was on location to check the status of this soon to be plugged well. Moore's service rig was spotted over the hole to the south and a pipe trailer was spotted to the north. There were no personell on location and several joints of tubing with rods were on the trailer and appeared to be cut. I spoke with Moore Well Services and they indicated that the plug job would be delayed as they will need to cut every joint of tubing out of the hole due to parafin build up. No violations.
-373869824 2013-10-24 I was on location to witness the plugging of this well. Plugging operations conducted by Moore Well Services and Petroset Cementing. Bullheaded bottom plug through 4.5" casing. Ripped pipe at 2640'. Set remaining plugs across the "Big Lime", Berea Sandstone and Ohio Shale, and Glacial Deposits. Good circulation was established prior to setting the last three plugs and cement circulated to surface. No violations. See plugging report for details on this job.

1829107457 2013-10-28 I was on location to check the plugs set on 10/24/13. George (Moore Well Services) and Don Caproletti were on location to get the pipe trailer off of location. The wellhead was buried to allow the equipment to be moved. George told me he topped the hole off with 5 bags of Sakrete on Friday. Don excavated the wellhead and the cement was at the top of the surface casing. There were no signs that the well was leaking. The 8.625" wellhead was spun off and the casing was far enough below grade that it did not require cutting. The ID was placed on the casing and the well was buried. This well has been properly plugged and abandoned. No violations.
