-1554684092 | 2012-02-02 | FOLLOW UP TO NOV # 885461286 My inspection this day due to request by the landowner, Doug Emrick, to inspect for violations. He has stated that Northwood told him more than a year ago that they would be plugging the well and still have not done it - now they are trying to produce it. He reports a spill at the tanks (2 100 bbl) where tank was run over and oil inside the dike area. I found dike area full of fresh water and hard to tell how much oil, if any, was run into the dike area. The 2 l00 bbl tanks have a large dike area with separator and meter inside - the water is so deep that I can't get to meter but looks like chart is old and has run around more than once. Dike appears to meet SPCC requirements but should be kept free of fluids. Identification posted on the tank is complete but has an incorrect permit number of #3974. The well is equipped with a pumping unit and has peat spread on ground around the well with a 6 x 6' brine kill area below the well - valves open at the well to tanks. Annual production has been turned in for 2010. The dike area needs drained and correct permit number needs posted on identification sticker - needs to have current chart to be considered producing.

189568377 | 2013-04-30 | Area still does not meet final restoration status. The well location has passed inspection however the tank battery was shared and Stonebridge has agreed to take care of filling, grading and seeding the dike. I called Dave Crippen and he is still trying to accomplish this task. He will talk with the landowner about getting some dirt from the property to fill dike. 8/21/2013: John Schneider with Stonebridge left a message stating he would be able to get the dike filled next week.

513306267 | 2012-10-17 | Inspection of well location revealed a graded but barren area which needs to be restored to state specifications. Diked area still exists with production equipment tangled in overgrown vegetation. Diked area also has oil and water contamination on surface all around and beyond the 1 100 barrel tank. Tank is empty and all flow lines have been disconnected. Meter is still hooked up but inactive and landowner's renter stated he no longer receives house gas. All production equipment needs to be removed and diked area needs to be cleaned of contamination followed by restoration. I called Northwood and talked to Bill and he said it should have been taken care of and will look into it. Failure to comply may result in a forfeiture of Bond. | 9-1-03C

994267390 | 2012-12-27 | Site still does not meet final restoration due to barren area at well head location, as last reported, however site is no being used for cattle grazing. I was told the tank battery was shared with Stonebridge for well 24120 and dike has yet to be filled and restored. I will come back for a follow-up inspection of the dike and determine final restoration status for this particular well once I have made contact with Stonebridge.
