1204113719 2008-02-26 I received a call from Salem office at 9:37 AM about a complaint of dumping paraffin and covering it up by the workover rig, The call came from Mr. Homer Sanor, 17665 Dr. Beloit OH, 330-682-6773. I called Mr. Sanor and told him I would meet him by 11:30AM. I arrived at 10:45AM. We proceeded to the well site where the workover rig was performing there work. Mr. Russell Miller, Vanguard representative was on location. I talked to him about hauling the paraffin away, vacuuming the water out and filling hole back in. This was farm ground. Mr. Russell stated that he could do that. He would call me when he had everything ready haul the paraffin, water out of the pit. I received that call later in the afternoon and witnessed the hauling away of the paraffin and the vacuuming out of water in the pit. The pit was closed. This case is resolved.
1248089676 2009-07-17 I received a call from Mr. Kevin Artchy(330-428-5460) of Ellete road in Beloit. He was concerned the parafin was being buried on his property. I called Russel Miller from Vangard(330-506-8152). I asked Russell if he could pick up the parafin and haul it away. He stated that he could do that. I arrived on location later that afternoon and found the pit was cleared of parafin and buried. Russell stopped by and we talked about the problem. He would call me the next time the rig comes in to service the well. Russell also stated that he would bring more top soil in to the pit. This case is resolved.
535943864 2005-12-28 The tank battery is shared with permits: 1549 and 1642 and consists of 3-210 bbl storage tanks, 3-separators, 2-1,400 gallon plastic salt water tanks and a dike. The wellhead identification is missing.

587562181 2007-12-13 I performed a random field inspection. Both the well and the tank battery were GPS'd. The ID sign was posted and the tank battery was in good shape. There were no violations at this time.
