1370614451 2013-06-05 I received a phone call from the Uniontown Office ( Wendy Booth ) about a leak at the above location. She gave me his phone number and address. I called John Curtis he said the leak was about 80' to 100' from the well head. I called Joe Tippert Field Supervisor from EnerVest he had two of his field personal meet me at the location. We walked to the well head there was a leak. The area has a oil stain 60' long 12' wide at one point no running oil. This is an open flow well. The stain was caused by a mist over a period of time. They will be cleaning the contaminate area and properly disposing of the soil.
1372172456 2013-06-05 I was notified by Wendy Booth from our Uniontown Office about a gas leak and oil at the property of John Curtis 18976 Middletown Rd. North Benton Ohio. I called John at 330-584-2931 and spoke with him, he said it was the T Herren #2. I called Joe Teppirt for EnerVest, he had one of his pumpers to meet me at the location. John Curtis was also there. This is a swab well, there is dry oil on the ground and gas leaking. This has been there for sometime. I instructed them to remove the contaminated soil and properly disposal of it and repair the leak.
1372174814 2013-06-21 EnerVest contracted Sweet Meadow Excavators to remove and dispose of 9yds of contaminated soil from the above location, they also hauled 9yrd.of new soil to fill in the area where the contaminated soil was removed. At this time there is no violations.
