| I inspected this producing unit as part of routine inspection activity. The wellhead and battery are located in a wooded area in a predominantly agricultural portion of Lake County. Access to the well and battery are afforded via an unpaved road which travels west from County Line Road. The lease road is aged as evidenced by some wash-outs and ruts, but is passable. The battery is comprised of one 100-barrel steel production tank and separator in a chain-link enclosure. The well is located an additional 1,000 feet north of the battery. The access road to the wellhead is overgrown and passable by 4-wheel drive vehicle with difficulty when dry. Connections and fittings at the wellhead and battery are secure and free from signs of leakage or problems. I observed  two discarded storage tanks (one 210-barrel and one 100-barrel)  which had formerly been in use at this battery but have been left on-site. I notified Broad Street that all unused equipment must be removed within 30 days.

| The leaking tank previously noted in the battery has been replaced with a newer tank. The remote pump jack requires an ID placard. Broad Street has been advised about the issue.

| The producing unit consists of a wellhead with pump jack and storage tank battery. The wellhead and battery are in chain-link enclosures. There is no proper ID at the wellhead which is located remote from the battery. The battery is comprised of two above-ground storage tanks (ASTs), 1 210 barrel, 1 100 barrel, and a mechanical separator. Both of the tanks are deteriorated significantly and failing at the bottom. The gravel material surrounding the tanks is saturated with free-phase oil. I contacted Ed Butler and advised that some remediation will be necessary. He indicated that they would evaluate and begin mitigation within the next two weeks.
