An inspection of this well revealed that the bottom backside of 100 bbl oil tank has corroded and contains a leak. Old oil and stained ground exists inside the earthen dike approximately 15' x 5'. The 200 bbl horizontal saltwater tank contained substantial corrosion around a valve and leaking. A bare area containing salt crystals extends from source outward, inside the dike, approximately 10' x 2'. Both tanks will need to be replaced and all contaminated material removed from the location. Note: well owner currently has charcoal drum placed on top of 100 bbl tank for filtration, due to similar complaints previously filed with the Division. A crude oil smell was verified therefore, the well owner shall install, on tank(s), activated charcoal filters on the vent stack and replace filters as needed. Vent stacks shall have functioning low-pressure relief. Tank hatch lids shall have a functioning seal and the hatch shall be sercured at all times the well owner is not on-site. Pollution and contamination also exists at the well head, due to stuffing box leak. The well and related equipment is located in Falls Township, which is considered to be an "Urban" area. For tank replacement, the well owners attention is directed to the Ohio Administrative Code 1501: 9-9-05(E) Urbanized Areas. The placement of new tank(s) must comply with this Chapter. 9-1-07

1299419018 2013-09-09 Status check to ensure Doug Kitchen is working on bringing up the well to ensure the gas does not escape from the oil tank. The waster in the dike has been removed. New charcoal has been put in the filter. The gasket on the lid on the oil tank still needs replaced. I gave Doug until 09/13/13 to finish. I have been in contact with Joan Hamilton who registered the complaint.
1406359019 2008-08-28 A complaint was received by the DMRM via email. The complaint alleges that gas from this well is burning eyes and throat. The owner, Chieftain Energy, was contacted and informed of the complaint. An inspection was conducted. During the inspection, no gas was detected. A gas meter showed 20.6 and 20.7 O2 levels around the tanks, well and complainants residence. Chieftain Energy has installed new charcoal at the tank battery.

All contamination and pollution contained within earthen dike. The release was not continuous as the well was found to be pumping at time of the inspection. Chieftain Energy arrived on-site and turned off the well and began cleanup.

After 6pm, the complainant states that the odor resembles raw natural gas. An inspection revealed that Mr.Doug Kitchen would need to remove contamination and pollution at the tank battery. Charcoal filters will also be required. The complainant was informed of the inspection results. An attempt to contact Mr. Kitchen by telephone on 8/6/2007 was not successful. A message was left stating that a complaint had been filed and to contact me immediately regarding the inspection results. Another attempt to contact Mr. Kitchen on 8/9/2007 was not successful. On 8/10/2007, Mr. Kitchen contacted Mr. Brent Bear and stated that he would resolve the deficiencies. 9-9-05(A)9

At the time of the inspection, no gas was observed around the well site. General housekeeping around well-head as fresh oil approximately 3' x 3' from stuffing box leak was observed. At the tank battery location old absorbent pads and peatsorb still remain from cleanup inside the dike.

859174580 2013-09-06 I met with Joan Hamilton concerning a complaint of natural gas order. I contacted the well owner, Doug Kitchen. Doug met me on site at 1320 hours. The charcoal filter needed replenished, the gasket on the tank at the hatch was in need of repair. The brine tank has two 2 openings in the top that need plugs and there was some water in the dike. Doug agreed to have these issues resolved by 09/13/2013. I spoke with Joan Hamilton to let her know of my findings.
