0276411543 2015-09-25 Administrative Inspection No Violations Placed phone call at 9:24 AM to Tim McConeghey with Enervest Operating regarding complaint. Mr. McConeghey was made aware of the complaint concerning an oil/sulfur odor. Mr. McConeghey stated that he would install a charcoal filter on the tank battery.
0753248028 2015-09-23 Production Wells No Violations Inspect well in regards to complaint alleging an oil/sulfur odor in the area. The complainant, Mr. Daniel Downing, lives on the east side of Northridge Road and stated that the odor is strongest on cool stagnant nights. The well was equipped with a pumping unit and motor. Identification was posted at the well. The well was not pumping during inspection. The tank battery consisted of (1) 210 barrel tank and (2) 100 barrel tanks inside an earthen dike. This well produces into a shared tank battery with #5551. Identification was posted at the tank battery. The gas detector was used at the tank battery and did not detect any levels of LELs, H2S, or methane. It was observed that the tanks did not have charcoal filters on them. The company will be contacted regarding this inspection and charcoal filters will be recommended. No violations noted during inspection. Time of inspection: 5:32 PM. Weather: 79*F, Sunny

1283182043 2010-08-30 Production Wells No Violations The inspection revealed an open pit containing a small amount of fluid next to the well. There are some bent rods on location. The service rig just moved off the location today after a service job. A new pump was put into the well. I talked with Mark McConnahay with Enervest who was on location to check the wells and tanks. This well shares a tank battery with the Holbrook well, permit 5551. There is proper display of identification posted at the tank battery. The tank battery consists of two 210 barrel tanks and one 100 barrel tank. There is also two horizontal separators. They are all contained within an earthen dike. Mark said that both wells are pumped daily. There are no violations noted this date.
1634457443 2015-09-25 Administrative Inspection No Violations Spoke with complainant Mr. Daniel Downing at 2:53 PM. Mr. Daniel Downing was informed that the company was contacted and that they volunteered to install a charcoal filter on the tank battery. A follow-up inspection will be conducted at the tank battery to see when filter is installed.

A phone call was placed to Tim McConeghey. Mr. McConeghey was not sure if that tank was in use and said that might be the reason a filter was not installed on that tank. Mr. Coneghey said he would find out and let me know. Time of inspection: 9:15 AM.
